A reactor / turbine trip was initiated on 5/22/91 at 2222 when a non-licensed l- operator inadvertently actuated the local switch which opens the main generator i

output breakers.

Down Report w Reactor 2-91-007 EL HS A reactor / turbine trip was initiated 91-06 910522 F 66.7 H 3 when a non-licensed operator inadvertently actuated the local switch which trips open the main generator output breakers. The resulting transient activated the reactor trip circuitry related to reactor coolant system overtemperature protection.

Corrective actions to prevent recurrence are being established and will be discus-sed in the LER.

, ST-HL AE-3Foo PAGE G OF 6 PORVs and Safety Valves Summary Following the reactor / turbine trip that occurred on 5/22/91, the resulting transient caused Reactor Coolant System (RCS) pressure Le increase. Pressurizer spray valve 2 RC-PCV-0655C.was in manual control and closed due to excessive
leakage and the available automatic spray valve (2-RC-PCV-0655B) was not able to reduce the pressure. Pressurizer PORVs 2-RC-PCV-0655A and 2 RC PCV-0656A j opened at approx 1"ately 2335 psig. RCS pressure reached approximately 2423 psig.
