The information presented is as initially received without verification or evaluation and is basically all that is known by Region V staff on this date.

DEFECTIVE ENGINEERED SAFETY FEATURES SYSTEM ELECTRICAL CONNECTORS On April 29, 1986, during an inspection of the multi-pin connectors used in the balance of plant (BOP) engineered safety system control circuits, the licensee discovered that the pin engagement of the connectors may be insufficient and may cause an open circuit when the wires of the connector assembly are moved. The inspection was conducted as a follow-up to the loss of power initiation reported in the PNO-V-86-24. The condition may result in engineered cafety features iroperability during a seismic event. This condition is generic to all three units.

Based on this finding, the licensee declared both trains of BOP engineered safety features inoperable. Unit 2 was in Mode 3 at the time of the finding and is in the process of cooling down to Mode 5. Unit 1 is currently in Mode 5 for a maintenance outage. Unit 3 is still in the initial phase of preoperational testing.

The licensee is evaluating, with the vendor representatives of the affected control equipment, what modifications are necessary to correct the problem.

The resident inspectors are following this matter closely.
