However, the transmittal forms which accompany these transcripts do not list the number of insert pages. Without this information we cannot l confire the copying cost charges for this material as shown on the invoices.  ;

It is requested that you confirm the number of insert pages in the transcripts J taken at Indian Point on March 22-25, 1983. Please provide this information directly to this office.

In the future, please insure that the transmittal sheets are completely and accurately filled out.
