NRC INSPECTION REPORT NO. 50-461/97014(DRP)

Dear Mr. Cook:

On July 7,1997, the NRC completed an inspection at your Clinton facility. The enclosed report

presents the results of that inspection.

.

During the pe'iod covered by this inspection, we noted continued decline in the quality of

maintenancI activities that occurred during the last two inspection periods. Specifically, poor

maintenance work practices have resulted in damage to plant equipment and inadvertent

actuations of plant systems.

We are also concemed about the four violations of NRC requirements described in the enclosed

Notice of Violation. In the first instance, three examples of procedure non-compliance regarding

the uae of load impact lists, the performance of maintenance activities, and the implementation

of modifications were identified. These violations are of concem because they represent

additional examples of a well documented problem area involving procedural compliance. In the

fourth violation, NRC inspectors identified that the minimum illumination level in the protected

area failed to meet regulatory requirements and your staff had not effectively implemented

programmatic controls designed to ensure protected area illumination requirements were

satisfied at all times.

These violations are cited in the enclosed Notice, and the circumstances surrounding the

violations a.e dscribed in detailin the enclosed report. Please note that you are required to

respond to this letter oc.d should follow the instruction specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to determine svhether further

enforcement actions are necessary to ensure compliance with regulatory requirements.

In addition to the routine resident inspection at Clinton power Station, the enclosed report

documents the resutts of a specialinspection conducted by members of the NRC headquarters (

staff from the Office of Nuclear Reactor Regulation.

1The results of that specialinspection provide insight into the causes of multiple violations

associated with human performance and procedural adherence that occurred at Clinton Power

Station between September 1996 and February 1997, as documented in several NRC inspection -

reports. The spe.cial inspection revealed weaknesses in the oversight of activities by :

management, the safety culture of Clinton staff and management, and the teamwork and

- communications among departments and between Clinton management and staff. These and

other causal factors will be further developed and refined through implementation of the ongoing

Clinton Integrated Safety Assessment and NRC Special Evaluation Team,

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosures will be placed in the NRC Public Document Room.
