SUBJECT: CLINTON INSPECTION REPORT 50-461/99014; 50-461/98012(DRP)

Dear Mr. McElwain:

On September 8,1999, the NRC completed an inspection at the Clinton Power Station. The

enclosed report presents the results of that inspection.

~ During the inspection period, activities at the Clinton facility were conducted in a safe manner,

and a safety-conscious approach to operationalissues was maintained. However, an instance

was identified where main control room operators failed to identify a discrepant switch position

and associated annunciator. - Specifically, when a control room ventilation chiller unit was

removed from service, operators failed to place a control switch in the required position to light

an annunciator. This condition went unnoticed through most of one shift and a control board

turnover until the issue was raised by the inspectors. This issue illustrated the need for

continued vigilance in the areas of operator attention to detail and control board monitoring. i

Based on the results of this inspection, the NRC determined that two violations of NRC j

requirements occurred. The first violation involves the operators' failure to place a control  !

switch in the proper position to identify a piece of out-of-service equipment. The second i

violation involves the adequacy of your Conduct of Maintenance and Safety Tagging

' procedures. These violations are being treated as Non-Cited Violations (NCVs), consistent

with Appendix C of the Enforcement Policy. These NCVs are de:cribed in the subject

--inspection report.~

Also during this inspection period, the inspectors reviewed 29 older licensee event reports.

Based on the results of these reviews, the NRC has determined that 19 violations of NRC ,

requirements occurred. All of these violations are being treated as Non-Cited Violations,

consistent with Appendix C of the Enforcement Policy. These Non-Cited Violations are

~ described in the subject inspection report. However, these Non-Cited Violations are not

described in the Executive Summary and will not be entered into the Plant lasues Matrix

because they involve events and associated issues that are at least 1 year old.

. If you contest the violations or severity level of the NCVs described in this report, you should

. provide a response within 30. days of the date of this inspection report, with the basis for your

denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington,

' DC 20555-0001, with copies to the Regional Administrator, Region lil, and the Director, Office

of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.
