NRC INSPECTION REPORT 50-461/99004(DRS)

Dear Mr. McElwain:

On April 7,1999, the NRC completed an operational readiness team inspection at the Clinton -

Power Station. The enclosed report presents the results of that inspection.

The team evaluated the effectiveness of the Clinton Power Station (CPS) operators to safely.

operate the facility. The two weeks of onsite inspection focused on your operational activities

. and included two separate periods of continuous 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> of control room and plant

- observations by NRC inspectors.

t During the first week of the onsite inspection, the inspectors noted some deficiencies in control

,

. room supervisor command and control, operator response to alarms, and work group supervisor

l review of work packages and questioning of work group personnel.' However, the inspectors

informed your management staff of these deficiencies and by the second week of the onsite

inspection, performance in these areas had improved. In particular, operators appropriately

used and adhered to procedures, conducted informative shift tumovers and briefings, promptly

recognized degraded plant conditions, correctly implemented technical specifications, effectively

communicated,' and understood the plant status. As a result, the NRC concluded that your .

operators were capable of safely operating the plant. . However, the recent improvement in these

and other operational performance areas is attributable to intense management focus and

short-term corrective actions; therefore,' continued long-term management focus is necessary to

.;

l ensure operational performance improvement is sustained.

,

In addition, the inspectors reviewed the activities associated with NRC Manual Chapter 0350

2 Case Specific Checklist Restart item IV.1, " Establish Program to Reduce and Maintain Control

.

Room Deficiencies." To evaluate this item, the inspectors reviewed existing operator

workarounds, operator challenges, and main control room (MCR) deficiencies. The inspectors

.

concluded that workarounds, challenges, and MCR deficiencies which would not be resolved

_ prior to unit startup would not significantly complicate plant operations and that adequate

. processes existed to manage and address these items.~ Based on this, the inspectors

determined that your staff had adequately addressed this item and it was subsequently closed

by the NRC Oversight Panel.'

a control room supervisor who for several minutes did not provide proper oversight of control rod

movements during a surveillance test. The second violation, which was identified by NRC

inspectors, involved a failure to perform an audit of accessible temporary modifications during

the last month of the first calendar quarter of 1999. The third violation, which was identified by

'

NRC inspectors, pertained to an inadequate control rod scram time surveillance test procedure

that did not direct the withdrawal of subsequent rods in accordance with the existing plant

configuration. The fourth violation, which was identified by NRC inspectors, related to an

inadequate diesel generator surveillance test procedure that did not ensure acceptable

frequency requirements could be met.

These Severity Level IV violations are being treated as Non-Cited Violations (NCVs) consistent

with Appendix C of the Enforcement Policy. These NCVs are described in the subject

inspection report. If you contest the violations or severity level of these NCVs, you should

provide a response within 30 days of the date of this inspection report, with the basis for your

denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington,

DC 20555-0001, with copies to the Regional Administrator, Region 111, and the Director, Office of

Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, the

enclosure, and your response, if you choose to respond, will be placed in the NRC Public

Document Room.
