NOTICE OF VIOLATION Connonwealth Edison Company Docket No. 50-374 As a result of the inspection conducted on May 10 through June 11, 1986, and in accordance with the General Policy and Procedure for NRC Enforcement Action (10 CFR Part 2. Appendix C), the following violations were identified:

1. Technical Specification 6.2.A.1 requires detailed written procedures be adhered to including procedures for plant start-up required by Appendix
"A", Section 4 of Regulatory Guide 1.33, Revision 2, February 1978.

Licensee Procedure LGP 1-1, " Normal Unit Start-Up", Paragraph F.2.h, requires control rods to be withdrawn in accordance with the approved rod sequence provided by the nuclear engineer.

Contrary to the above, on May 26, 1986 an operator failed to withdraw a control rod in accordance with the approved rod sequence provided by the nuclear engineer. Specifically, Unit 2 Control Rod 10-19 was withdrawn from Position 12 to Position 48 instead of to the required Position 24.

This is a Severity Level IV violation (Supplement I).

! 2. Technical Specification 6.2.A.1 requires detailed written procedures be adhered to including procedures for abnormal, offnormal, or alarm conditions required by Appendix "A", Section 5 of Regulatory Guide 1.33 Revision 2, February 1978. Licensee Procedure LOA RD-03, "Mispositioned Control Rods", Revision 5, Section D.2 provides instructions for a control rod withdrawn beyond its in-sequence position. This procedure requires the o Thermal Power)perator , 00-7 Option to 2demand (Controlprocess Rod Position), computer and printouts OD-8 (Local0D-3 (Core

. Power Range Monitor Readings) and then to consult the nuclear engineer for the method of returning the mispositioned rod to its correct in-sequence position.

Contrary to the above, on May 26, 1986 the operator failed to demand the

! required process computer printouts and failed to consult the nuclear l engineer prior to returning the mispositioned control rod to its correct in-sequence position.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation
(1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violation; and (3) the date when full compliance will be achieved.

Consideration may be given t6 extending your response time for good cause shown.
