l NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos. 50-373; 50-374 LaSalle County Station License Nes. NPF-11; NPF-18 i,1its 1 and 2 During an NRC inspection conducted on September 8 through beptember 12,1997, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:
Technical Specification 3.5.2 required that at least two of the following shall be operable when the plant is in operational condition 4 or 5:
a.
Iow pressure core spray system b.
Iow pressure coolant injection (LPCI) subsystem "A" of the residual heat removal (RHR) system c.
LPCI subsystem "B" of the RHR system d.
LPCI subsystem "C" of the RHR system e.
the high pressure core spray system Technical Specification 3.5.2, Action b, required that with both of the above required subsystems / systems inoperable, suspend CORE ALTERATIONS and all operations that have a potential for draining the reactor vessel. Restore at least one subsystem / system to OPERABLE status within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> or establish SECONDARY CONTAINMENT INTEGRITY within the next 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.
Contrary to the above, on September 4,1997, all of the above systems were inoperable and the licensee was unabls to restore one subrystem/ system within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> or establish secondary containment iniegrity within the following 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.
This is a Severity Level IV violation (Supplement 1) (50-373/97015-01).
Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison Company is hereby required to submit a written statement of explanation to the U.S. Nuclear Regu:atory Commission, ATTN:
Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region 111, and a copy to the NRC Raident inspector at the LaSalle facility, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the resuls achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the < lie when full compliance will be achieved. Your response may reference or include previous s. icketed correspondence if the correspondence adequately addresses the required response. If an adequate reply is not received within the time specified in this Notice, an Order or a Demand for Information may be issued as to why the license should not be modified, caspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
Because your response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprieary, or safeguards information so that it can be placed in the PDR without redaction.
