f40TICE OF VLOLAT10!1 Commonwealth Edison Company Docket llo. 50-374 LaSalle County Station Unit 2 License lio. f4PF-18 During an f1RC inspection conducted on IJovember 2 through 9, 1993, violations of f4RC requirements were identified. In accordance with the " General Statement of Policy and Procedure for fiRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below.

1. Technical Specification 6.2.A states that written procedures shall be established, implemented, and maintained covering the activities recommended in Appendix A, of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Revision 2, February 1978 recommends that written procedures for draining of the Reactor Water Cleanup system be established.

LaSalle Operating Procedure RT-12, "RWCU System Draining" step F.5.b requires verification that the A/B/C filter demineralizers have been backwashed as per other operating procedures prior to removing the system from service.

Contrary to the above the Reactor Water Cleanup System was removed from service on or about October 1, 1993, without performing a backwash of the "B" filter demineralizer.

This is a Severity Level IV violation (Supplement I).

2. Technical Specification 6.2.B states that radiation protection procedures shall be established, maintained, and adhered to.

LaSalle Administrative Procedure LAP-100-22, section F.2.e.4, states, in part, that it is the responsibility of the individual worker to comply with the requirements of the Radiation Work Permit and all associated documents.

Contrary to the above:

a. On October 31, 1993, an operator encountered a dose rate i field which caused his electronic dosimeter to alarm and did {

not immediately leave the area as required by the Radiation l Work Permit under which he was working.

b. On October 31, 1993, an operator entered a contaminated area which contained levels of contamination greater than 50,000 dpm/100cm' wearing only minimal protective clothing. The Radiation Work Permit did not allow for the wearing of minimal protective clothing in contaminated areas greater l than 50,000 dpm/100cm'.

Notice of Violation 2 This is a Severity Level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATIN: Document Control Desk, Washington, D.C. 20555 with a copy to the U.S. Nuclear Regulatory Commission, Region III, 801 Warrenville Road, Lisle, Illinois, 60532-4351, and a copy to the NRC Resident Inspector at the LaSalle County Station within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or a demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
