AUG 31964 2.0 PlRPOSE AND SCOPE This instruction supplements Reference 1-A and provides the inspection criteria and documentation requirements that shall be used to perform inspections of cable tray support and associated component fabrication and installation in Unit II Category 1 buildings and areas including roon #'s listed below in Auxiliary and Control buildings.

The Unit II PFG shall issue separate work packages for cable tray supports in accordance with Reference 1-8 for Unit II

( Category 1 buildings and areas. Unit 1 PFG shall issue work packages in rooms listed in paragraph 2.0.

The QC Inspector shall perfonn inspections according to the sequence of operations listed on the Operation Traveler (Attachment 1 and 2) in accordance with Reference 1-C and the I acceotance criteria specified in this instruction.

3.1 FABRICATION AND INSTALLATION OF SUPPORTS ~

3.1.1 Material Traceability Cable tray supports and associated components do not require material identification or a MR for materials conforming to ASV4 A-36 and/or A-500 gr.B. No substitution of material will be allowed unless specifically noted on the design drawings and/or desian changes.

3.1.2 Subdivision of Material __.

QC shall perform a surveillance inspection to verify the transfer of "QP" numbers in accordance with Reference 1-0, when subdividing bulk steel which has already had protective coatings applied and is intended for use in the Reactor Building.

3.1.3 Bolting ~ ~ ~ ~~'

3.1.3.1 The QC Inspector shall perform the following ins pections:

a. Bolts are the correct length as indicated by having the bolts flush with, or outside of, the face of the nut when coupletely installed.
b. Turning elements are on the correct face.
c. Washers are used when required.

NOTE: hhere long slotted holes are used on an outer ply, structural plate washer or a continuous bar not less tharc 5/16" thick with standard holes shall be provided only when shown on the design drawing. ,

d. Bol t heads and nuts shall rest squarely against the metal or washer.
e. Where bolts are used on surfaces having slopes greater

( than 1,in-20 with a plane normal to the bolt axis, beveled washers shall be provided to give ful1 bearing to the head or nut.

f. When " Turn-of-nut" method is used for bolt tightening, the impact wrench or other pneumatic device does not require calibration. Howeve r, the device shall be of adequate capacity and sufficiently supplied with air to pe rfonn the required tightening of each bol t in approximately ten seconds. When this method is used, the following steps will be followed by construction:
1. All bolts in each joint shall first be brought to a " snug tight" condition (snug tight is defined as the tightnes's attained by a few impacts of an impact wrench or the full effort of a man using an ordinary spud wrench).
2. All the nuts shall then be match-marked using an approved industrial marker, i.e: Nissen.
3. All the bolts in the joint shall then be tightened to the applicable amount of nut rotation specified j in Attachment 3. j i,-
4. Tightening shall progress systematically from the l

- most rigid part of the joint to its freest edges. l

5. There shall be no rotation during tightening of the part not turned by the wrench.
6. The end of all bol ts shall be marked with the industrial marker when required nut rotation has been attained (ie Nissen). l
g. When several bolts are installed in a single joint, the QC Inspector shal1 assure -bolts previously tightened which may have been loosened by the tightening of subsequent bolts are tightened to the presc ribed amount.
h. Unless otherwise noted on the drawing, all connections using high strength bolts shall be bearing type connections wi th threads excluded from shear planes.
i. When field connections are to be made by use of A325 or A490 bolts and tightened wi th a calibrated torque wrench, a hardened flat washer shall be used under the tu rned element and shall be tightened as fol lows :

NOTE: Two hardened washers shall be used with all A490 bolts used to connect material having a specified minimum yield point less than 40 KSI (i.e., A36).

j. As an alternate to the above, for tightening of bearing type connections using high strength bolts, see Reference 1- F .

k. A490 and Galvanized A325 bolts shall not be reused.

Other A325 bolts may be reused if approved by Engineering using permanent plant docunentation (i.e. ,

DCA, CPPA, Traveler, etc). I i

1. ASTM A-449 bolts may be sut tituted for A-325 bolts I (tighten per the requirements for A-325).

m. ASTM A 307 bolts shall be tightened to a " snug tight" condition.
n. The QC Inspector will apply toque seal to all satis-factory tightened nuts / bolts.

3.1.4 Supoort Verification 3.1.4.1 Cable Tray Drawings FSE map drawings have been prepa red by the Engineer to identify the cable tray support with a unique number. If more than one suppo rt is at a specific location, the unique number will be determined by each elevation.

FSE-00159 detail drawings have been prepared for specific hanger fabrication. The FSE-00159 provides the basic support type as illustrated on the design drawings. The design drawings and design changes required to perfo nn fabrication, installation and inspection will be noted on Attachment 1 and- verified by Civil Engineering as being the only design drawings and/or design changes requi red for i

inspection. All current revisions used for inspection will be noted on Attachment 1 (including inspection procedures and instructions). The QC Inspector shall use the FSE-00159 only for location of cable tray hanger, verify hanger unique number, and hanger elevation unless directed by the engineer with a Design Change Doctsnent. Again attachments to concrete or embedded plates shown on FSE-00159 are for reference only.

The detennination of detail to be used will be in accordance with design drawings and design changes noted on Attachment 1.

If the engineer has provided the installation criteria for a support on a Design Change document, the QC verification should be per the Design Change drawing, disregarding the FSE-159.

NOTE: FSE-00159 detail drawings issued prior to 5-26-81 may not show the location of the support, and may not necessarily be revised to add the location of the support. In cases where hangers are stacked and no locatior. is provided on the FSE-159 series drawings, Engineering will provide location coonfinates and elevation on appropriate design drawing. In cases where hangers are not " stacked" the FSE map drawings may be used for verification of hanger identification.

- CPSES - Page 6 of 28 AUG 311994 3.1.4.2 Configuration The QC Inspector shall verify the coupleted support is in accordance with the design drawing and applicable design change docunents. This verification shall include the following attributes:

a. Support type
b. Support Member (s) shape (i .e. , angle , channel , etc.),

and size

c. Location of members in accordance with design drawing dimensions including " work points" and bolt locations.
d. Support members shall be ins talled plumb & level per Reference 1-G.

3.1.4.3 Use of Drawings for Rework Individual hangers (those shown on FSE map drawings as not

" stacked" vertically) that are reworked by the craft do not

(- nonnally require use of the FSE-00159 drawing by the QC Inspector for verifying the hanger unique number for inspection of rework. In any case, where the proper unique number is' considered questionable (i.e., not readable, covered or improperly stamped), then the FSE-00159 shall be used for verification.

" Stacked" hangers that are reinspected shall have the unique number of the hanger verified with the FSE-00159 drawings.

Once the hange r is identified by the FSE-00159 drawing ,

physical identification of the unique number on the hanger is not necessary , and ins pection documentation shal l reference the applicable FSE-00159 drawing. Hangers that are reinspected shall not have their unique numbers restamped unless directed by the Quality Engineer and/or Quality Control Supe rvi sor.

3.1.5 Verification of Baseplates for Grouting An Inspection Report indicating bearing (" Satisfactory or '

Unsatisfactory") shall be canpleted as required per Reference 1-H.

- CPSES - Page 7 of 28 AUG 3119M 3.1.6 Welding Inspection Welded cable tray supports and associated components shall be inspected for acceptance per Reference 1-I. All attach-ments to embedded weld plates shall be per design drawings.

In areas where specific criteria are not given on design drawing, Attachment 5 will be used. If a conflict exi sts I between Attachment 5 and the design drawing, the design I drawing shall take precedence.

3.1.7 Plug Welding Unused holes that are not acceptable "as is" per the design drawing and/or design changes and misdrilled holes will be plug welded per Reference 1-J.

Plug welded holes shall be inspected using the acceptance criteria of Reference 1-I.

3.1.8 Design Change Verification

( The QC Inspector shall recond all design change docmentation .

(OCA/ CMC) and the revision thereof used for inspection on the Operations Traveler and/or Inspection Report fo r each applicable inspection operation completed.

3.1.9 Solice Plates / Side Plates / Heavy-Duty Clips / Miscellaneous Tray Components NOTE: The design drawings and design changes required to perfom fabrication, installation and inspection will be noted on Attachment 2 and verified by Civil Engineering as being the only design drawings and/or design changes required for inspection. All current revisions used for inspection will be noted on Attachment 2 (including inspection procedures and instructions).

3.1.9.1 Wel ded splice pla tes, side plates, heavy-duty clips and mi scellaneous cable tray support components shall be inspected for final vi sual inspection of the weld using acceptance criteria of Reference 1-1 and Attachment 2.

l 3.1.9.2 Bolted cable tray clamps for connecting cable tray to hangers shall be inspected in accordance with following criteria and the use of Attachment 2: l

a. Friction type clamps shall be installed on transverse cable tray supports unless othemise noted and shall be inspected to meet the requirements outlined in Paragraph 3.1.9.2 b, c, and d.

b. Proper bolt ins talla tion and bolt ty pe, including connection of tray to clip and clip to hanger. See Attachment 4 for ASTM bolt type and marking, j
1. All A307 bolts shall be snug tightened and shall meet the applicable requirements of Pa ragraph 3.1.3.2 and should be " torque sealed" with Inspector's lacquer.
2. All high strength bolts (A-325 or approved substitute) shall be tightened per Paragraph 3.1.3.
c. Field fabricated clips meet dimensional requirements.

All clips, vendor supplied and field fabricated, are installed and drawn together after tightening.

d. All bolted connections shall be docunented on the l Operation Traveler.
e. Haavy duty tray clamps shall be installed on longitudinal cable tray supports unless otherwise noted and shall be inspected to meet the requirements outlined in Paragraph

( 3.1.9.2 b, c, and d.

3.2 REWORK TRAVELERS No rework travelers /IRN's are required for comple ted operations that require correction provided Reference 1-C and Reference 1-K are maintained. Explanation of corrected operations shall be documented on Attachment 1. Once the QC/QE review operation on Attachment 1 and 2 is signed of f, a rework traveler will be required. Any deviations from the above will require QE approval .

3.3 REINSPECTION OF PREVIOUSLY INSPECTED CABLE TRAY SUPPORT l All cable tray hangers (excluding framework in Unit II Cable Spread Room) and including vendor fabricated hangers installed in Unit II Category 1 buildings and areas including roans listed in paragraoh 2.0 prior to the issuance of this instruction shall be reinspected using Attachment I and the following welding inspection attributes (if applicable documentation denotes that welds have been previously accepted by QC):

A. Weld configuration per Gibbs & Hill drawing (veri fy with paint applied)

1. Type (fillet, flare bevel, etc.)

- CPSES - Page 9 of 28 AUG 31 BM NOTE: If weld configurations are acceptable, sign Attachment 1 and note " Acceptable per QI-QP-11.10-2A Section 3.3." If weld configurations are not acceptable, document on an unsat IR (Attachment 6) and fomard to QE for evaluation.

During the reinspection process, if any items are found to be unsatisfactory or inaccessable for inspection, they shall be listed on an unsat IR (Attachment 6) and forwarded to QE l for evaluation. l 3.4 INSPECTION OF PREVIOUSLY INSTALLED CABLE TRAY CLAMPS All cable tray clamps that have been installed in Unit II Category 1 Buildings and areas including rooms listed in paragraph 2.0 prior to issuance of Attachment 2 shall be inspected using Attachment 7. If any of the cable tray clamps in the above mentioned areas have been inspected per QI-QP-11.10-2 they do not need to be reinspected. Any unsatisfactory conditions will be documented on Attachment 7 and if any rework is required by Craf t, it will be perfonned

using Attachment 2. QC Inspectors will close out Attachment 7 and sign applicable operation (s) in Attachment 2.

3.5 SURVEILLANCE AND INSPECTION OF COMPLETED INSTALLATIONS FOR SEPARATION /SP ACING Surveillance ins pections of canpleted ins tallations fo r separation / spacing requirements from safety related items will be in accordance with Reference 1-L.

3.6 DOCUMENTATION ,

The QC Inspector shall docment the inspection of cable tray support and associated components on the Operations Travelers (Attachments 1 and 2). l Ins pection Reports shall be processed in accondance with Reference 1-M and section 3.3 of this instruction, when applicable, and Operation Travelers processed in accordance with Reference 1-C.

3.7 NONCONFORMANCES All unsatisfactory condi tions shall be docmented on an Inspection Report as shown in reference instructions and/or using Attachment 6 or 7 and/or an NCR per Reference 1-M and 1-N respectively as directed by the Quality Engineer and/or Quality Control Supervisor.
