APPENDIX A i NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458/85-73 Ri'er Bend Nuclear Station License: NPF-40 During an NRC inspection conducted on October 28 through November 1, 1985, violations of NRC requirements were identified. The violations involved failure to provide proper training for radiation protection technicians and failure to maintain training / qualification records. In accordance with the

, " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1985), the violations are listed below:

A. Failure to Provide Training

! . Facility Technical Specifications, Section 6.8.1, " Procedures and Programs," states, in part, " Written procedures shall be established, implemented, and maintained covering the activities referenced below:

The applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978."

l Regulatory Guide 1.33, Appendix A.7.e.(6) establishes the requirement to i implement procedures for training personnel in radiation protection. This

, is further amplified in River Bend Radiation Section Administrative Procedure (RSP) 0003, " Personnel-Qualification for the Radiation Protection Section," which establishes training and qualification requirements for radiation protection technicians. Paragraph 5.2.4 of RSP-0003, states, "An employee is designated as a Senior Radiation

Protection Technician after completion of the requirements listed for i Junior and Senior Technician . ...

Contrary to the above, the NRC inspector determined on October 31, 1985, that eight designated Senior Technicians had not been provided training l courses as required by Section I.B.11 of Attachment 3 to RSP-0003.

This is a Severity Level IV violation (Supplement IV)(458/8573-01).

B. Failure to Maintain Training Records Facility Technical Specifications, Section 6.8.1, " Procedures and Programs," states, in part, " Written procedures shall be established, implemented, and maintained covering the activities referenced below:

The applicable procedures recommended in Appendix A of Regulatory l Guide 1.33, Revision 2, February 1978."

2 Regulatory Guide 1.33, Appendix A.7.e.(6) establishes the requirement to implement procedures for training personnel in radiation protection. This is amplified in Plant Procedure RSP-0003, paragraph 7.3.1, which states, in part, "The Radiation Protection Technician Qualification Matrix is main +.ined current by the Radiological Engineering Supervisor / designee."

Contrary to the above, the NRC inspector determined on October 31, 1985, that the Qualification Matrix for eight Senior Radiation Protection Technicians had not been updated for periods in excess of 8 months.

This is a Severity Level IV violation (Supplement IV)(458/8573-02).

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office, within 30 days of the date of letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given-to extending the response time.
