SCOPE The purpose of this procedure is to assign the responsibilities and authority of the Shift Technical Advisor, and to define the admin-istrative controls, training, and documentation established to ensure that the Shift Technical Advisor (STA) can properly fulfill his assigned responsibilities.

3. ORGANIZATION 3.1 The Operations Superintendent is responsible for the implementation of the STA program.

3.2 The STA will NOT exercise operational authority over any personndl.

His is a purely advisory position.

4. STA PROGRAM The STA Program implements the Accident Assessment Program and portions of the Operating Experience Assessment Program at Davis-Besse. Section 5 and 6 detail these two functions.

5. ACCIDENT ASSESSMENT PROGRAM The first major function of the STA is to carry out the Accident Assessment Program at Davis-Besse. The purpose of the Accident Assessment Program is to provide additional technical and analytical capability, dedicated to concern for the safety of the plant, in the control room to support the diagnosis of off-normal events and to advise the Shift Supervisor on actions to terminate or mitigate the consequences of such events. The STA does not have any respon-sibility to manipulate controls or directly supervise any operations.

This section of the procedure addresses the STA's specific responsibili-9 ties concerning Accident Assessment and STA Qualifications.

5.1 Responsibility 5.1.1 The STA is required to report to the Control Room within ten minutes of being notified that any of the following events have occurred:

1. Any reactor or turbine trip.
2. Any unit runback or unplanned power transient.
3. Any loss of offsite power.
4. Any loss of safety system function, (when that safety system is being called upon to actually perform its function).
5. Any station emergency covered by Emergency Procedures or the Emergency Plan.
6. Any time a limiting condition for operation is exceeded per Technical Specifications.

The Shift Supervisor or his designee is responsible for ensuring the STA is notified immediately when any of the above events occur.

5.1.2 The STA will also be notified by the Shift Supervisor ,

or his designee prior to any of the following events, and the STA will be present in the Control Room when the event takes place.

1. Taking the reactor critical.
2. Power changes at >20 MWe/ minute.
3. Any other planned unusual or abnormal event.

3 AD 1839.04.8 The Shift Supervisor may also request the STA's assistance in evaluating any non-routine plant con-ditions required for testing or maintenance. The STA's presence at these events is considered an important part of his on going training.

5.1.3 The STA should be notified of any other significant events which could affect plant safety or performance, but do not require his presence in the Control Room.

5.1.4 When called upon by the Shift Supervisor, the STA is to evaluate the incident or problem to the best of his ability to determine that events are following the expected sequence. This will require the use of resources such as Emergency Procedures, Operating Procedures, Technical Specifications and the FSAR/USAA.

The STA should not hesitate to contact appropriate personnel for assistance. The STA should try to maintain a broad perspective of the event by observtas indications in the Control Room, and continually analyze the progress of the event so as to be able to advise the Shift Supervisor when called upon to do so.

5.1.5 The STA should keep the Shif t Supervisor advised as to where the STA can be contacted.

5.1.6 The STA will maintain a log of important events.

This is a non-operating log and is solely for the 8l use of the STA in monitoring plant status. The log will receive no review. The STA log will be maintatsad in the Station Files for two years in accordance with $

Records Management Administra-tive Procedure, AD 1841 04- V During the course of his shift he should record any events or evolutions he considers significant. He should also make note of any advice given with regard to a particular event and the outcome of the event.

4 AD 1839.04.9 9l Normally the STA should attend the 0800, 1600, and 0000 Shift Supervisor Turnovers. If Turnover

is missed, the STA should attend the Assistant Shif t Supervisor Turnover and/or discuss plant status with the Shift Supervisor. He will ensure he is aware of plant status and planned events.

This may require periodic visits to the Control 9 Room.and discussions with the Shif t ' Supervisor.

2. It may become necessary to arrange for an alter-nate relief in the event that the scheduled relief or on-shift STA becomes unavailable due to illness or injury. If this occurs, the STA Supervisor should be notified immediately so that he may arrange for a relief. An on-shift STA should not delay seeking necessary medical attention because he is the only STA on site.

The STA Supervisor should be notified and will arrange for an STA relief to assume the duties 9 as soon as possible. Technical Specification TS 6.2.2 requires no greater than a two hour period with no STA on site.

5.1.8 STA Required Reading To assist the STA in keeping abreast of possible 3 problems and operational performance, a Required ~3 Reading Log will be maintained by the STA. During review of the documents listed in 6.1.1 through ,

6.1.8, the STA conducting the review will evaluate the information for its importance in the continuing training of the STA's. Information deemed of importance or significant interest to the STA's will be entered in the STA Required Reading System. The STA will log the document on the Required Reading Log (Attachment 1) and enter it into the Required Reading Book. It is the responsibility of each STA to ensure he has read 9l all required reading when he assumes his shift. He should initial his completion of each required reading item on the Required Reading Log.

The log will receive no review. The STA Required

  • Reading Leg will be maintained in the Station Files for two years in accordance with Records Management Administrative Procedure, AD 1848.00. A copy of l completed log sheets will also be sent to Training i to document the required reading performed by the STA's. )

5.2 Qualifications In order for an individual to qualify as an STA, he must possess 6

5 AD 1839.04.8 9 3l the minimum of a bachelor's degree in a science or an engineering discipline. In addition, the STA candidate must successfully complete the Davis-Besse Shift Technical Advisor Training Program as detailed in AD 1828.21, Shif t Technical Advisor Training Program.

8 All qualified SIA's shall maintain their qualification in accordance with the.requalification portion of AD 1828.21, Shif t Technical Advisor Training Program.

6. OPERATING EXPERIENCE ASSESSMENT PROGRAM The second major function of the STA is to participate in the Davis-8 Besse Operating Experience Assessment Program. The purpose of this program is to evaluate plant operations from a safety point of view, and to ensure that plant personnel are aware of safety-related generic plant problems and trends in Davis-Besse performance that relate to their specific areas. At present, all Davis-Besse sections participate in the Operating Experience Assessment Program through the preparation of Licensee Event Reports, responses to NRC IE Bulletins, review of procedures, etc. The role of the STA is not to take over these duties, but to augment them by providing additional input to the existing assessment programs and by providing additional safety related information and recommendations to all Station sections.

8 This section of the procedure addresses the STA's qualifications and specific responsibilities concerning Operating Experience Assessment, and the interfacing of these responsibilities with the Nuclear Mission departments and/or their sections. This last item deals specifically with: A) the transmittal and resolution of comments and recommendations made by the STA and sent to the appropriate department or section head; B) the documentation of STA Operating Experience Assessment Program duties; C) the acquisition of safety-related 8l information by the STA from company and/or outside sources; and D) the means by which the STA portion of the Davis-Besse Operating Experience Assessment Program will be audited.

6.1 Responsibilities 6.1.1 Licensee Event Report (LER) preparation and review: An 8' STA may be assigned as coordinator for the preparation of an LER. If assigned, he will ensure technical accuracy and adequacy of the LER. An STA will also receive a copy of draft LER's sent out for review and comment per AD 1804.00, Reports Management, and the final LER. He will give special consideration to the 8 safety-related aspects of the LER. An STA coordinator for an LER will resolve and incorporate comments returned on the draft LER.

6 AD 1839.04.8 6.1.2 Davis-Besse Transient Assessment Program (TAP) Report Preparation and Review: The STA's will review the 8l completed TAP report and make recommendations to the appropriate section or department head, if necessary, concerning the safety-related aspects of the report.

6.1.3 Review of TAP Reports from other B&W 177 Operating 81 Plants: The STA's will receive a copy of each TAP report generated at the other B&W 177 plants from the 8 Station Technical Section. An STA will review each report and make recommendations to the appropriate section or department head, if necessary, concerning the safety related aspects of the report.

8l 6.1.4 Surveillance Test Schedule Review: The STA's will receive an up-to-date copy of the Surveillance Test Schedule Computer Printout from the Station Technical Section. This schedule will be updated and reviewed daily to ensure that required tests are completed on time. Any problems will be resolved with the appropriate section head or Shift Supervisor during the STA's shift, if possible, or passed on during STA turnover for subsequent resolution during the following STA shift. The Surveillance Test Engineer should always be informed of any discrepancies in ST scheduling.

6.1.5 Institute of Nuclear Power Operations (INPO) Sinnificant 8 Operating Event Reports (SOER's): The Nuclear Licensing Group will receive SOER's and route them to the STA's.

An STA will be assigned to evaluate the SOER's applica-bility to Davis-Besse, and to write a response to file. The evaluation will determine if any actions are required, and the response will specify the resolution for each recommendation. Any recommendations that are already being complied with, are not applicable to Davis-Besse, or are fully resolved by the STA should be identified in the response as being closed-out.

Any recommendations that are being assigned to other sections or departments for action / resolution will be identified by number and responsible group in the STA's response. In the cases where the evaluation ,

identifies actions to be performed by other groups, notification menos will be sent by the STA to the responsible sections or departments. These menos will indicate which recommendations the assigned group is to resolve and will require a written response from each group, with a copy to be sent to the STA. 'A copy of any such memo will be sent to Nuclear Licensing, who will issue formal commitments requiring the actions and response requested by the STA's meno. The n

7 AD 1839.04.8 STA will review all required responses to ensure i adequacy. The STA's initial evaluation and response will be reviewed for adequacy by the STA Supervisor or his designee, with this review documented on the response. Copies of all STA responses are to be sent to Quality Assurance, Licensing, the Station Commitment Tracking Clerk, and Nuclear Safety Department.

6.1.6 Nuclear Network Information Review: The STA's will receive weekly Nuclear Network reports containing Significant Event Reports (SER's), Operating Plant Experience Reports, and INPO Operations and Maintenance Reminders. The report will be assigned to an STA who will review it for nuclear-safety related information applicable to Davis-Besse. .In most cases these reports will be of limited value and will not require actions to be taken. These reports will be sent to the appropriate Section Head for information and for any actions he desires to take. There will be no tracking of any information transmitted in this way.

The STA review of SER's is to be logged, by SER number and disposition, in the STA Document Review Log. The STA Supervisor will periodically review selected SER's and their resolution. This review will be documented by initialing the margin of the Document Review Log by the SER's reviewed. In the unusual event of an SER of such significance that important corrective actions need to be taken, that SER will be . entered into the SOER tracking system and will be handled like an SOER.

This will be documented in the SER section of the Document Review Log.

6.1.7 NRC IE Bulletin, Circular, and Information Notice Review: The STA's will receive, from Nuclear Licensing, 8l a copy of NRC IE Bulletins, Circulars, and Informatics 8l Notices. An STA will review the NRC Bulletins, Circulars, or Information Notices and make recommenda-tions and/or comments on the safety related aspects of the notice, if necessary.

8 6.1.8 SRB Amenda: The STA's will receive a complete SRB

  • Agenda each week from the SRB Clerk. An STA will review the material and make recommendations to the appropriate section or department head, if necessary, concerning the safety-related aspects of the material.

6.2 Transmittal of Significant Safety Related Information 6.2.1 The STA will send significant safety-related information or other important information, to the appropriate

8l section or department head for his use in training his staff or for incorporation into his section's policies and procedures.

8l 6.2.2 The STA will also send information, when appropriate, to the Training Manager for inclusion in the on going training programs of the Davis-Besse Maintenance, Chemistry and Health Physics, Operations, Guard Force, and Administrative and Clerical Personnel.

6.3 Qualifications 6.3.1 The STA Qualifications for Operations Assessment will be the same as those for Accident Assessment.

6.4 Administration of the STA Operating Experience Assessment Program Responsibilities 6.4.1 Transmittal and Resolution of Comments and Recommenda-tions made by the STA:

8 The methods available for transmittal and resolution of comments and recommendations are outlined below.

1. The STA may, at his own discretion, transmit his comments and recommendations through consulta-tions with the section head involved. If the STA feels he is not receiving adequate cooperation or feedback from the section involved, he may then transmit his comments and recommendations through the use of the following documents:

2. The selection of the document used will depend on the type and the significance of the comment or recommendation. The STA must use good judgement in his document selection to ensure that the comments are dealt with in the most efficient manner and to avoid burdening a section with un-necessary paperwork.
3. Because the Intra-Company Memorandum has no inherent means of generating feedback, it should be used only when the other controlling documents
7) are not appropriate for the situation. It is the responsibility of the STA to follow-up on response requests to ensure that they are received.
4. When comments or recommendations indicate the need for a change to a procedure or for FCR 7l. preparation, the STA can write it himself if he feels sufficiently qualified to do so. If the STA fee}s unqualified to write the procedure modification or FCR, he will transmit a recommenda-tion to the proper department or section head requesting the change. The STA will assist the individual assigned the task of writing the procedure modification or FCR in any way he can to ensure that the task is completed.
5. When procedure changes are involved, Major Modification Requests are the preferred method for transmitting recommendations and comments.

This ensures that the responsible section head can review and respond to STA comments before changes are incorporated in the procedure. If the recommendations or comments are of such a nature 'that procedures must be immediately revised, the Temporary Modification may be used.

6.4.2 Documentation of Reviews, comment and recommendation transmittals, and resolution of comments and recom-sendations.
