On October 23, 1981, I briefed RIV Management relative 'to this matter and i
i stated that as a minimum HL&P should have at least documented the basis of this work proceeding in meeting minutes.
Since the Quadrex Report questioned the assumptions of loading civil structures and the plan to verify actual l
loading after the structure has been erected and since the concrete place-ments proposed are irreversible, this one area of work concerned me, in that, it appears that even a crude risk assessment was not docimented.
