Catawba Nuclear Station Blast and Coat Drinking Water Line SCDHEC Construction Permit Approval File: CN-211.30 Attached please find the approved construction permit to install the drinking water line to serve the Blast and Coat Facility at Catawba.

Please note that any changes to the approved plans and specification may require additional approval from SCDHEC prior to implementation of the changes.

Additionally, this approval is for construction only and does not authorize this system to be placed into operation. Prior to operation of the system, an operational permit will have to be applied for and obtained before the system can be placed into service.

Dear Sirs:

Enclosed is your permit to start construction on the Blast &

Coat Facility-Catawba Nuclear St. water supply system. Unless construction is completed by the expiration date, it will be necessary to reapply since this permit will no longer be valid.

This permit is for construction onl_y, a permit to operate will have to be issued by the Department before the system can be placed into operation. If changes (line size or length, addition of treatment, addition of fire hydrants, etc.) are made during construction from the approved plans and specifications, prior approval must have been obtained from the permitting section. If minor changes (valve location, moving the fire hydrant a short distance, etc.) are made during construction from the approved plans and specifications, these changes must be shown on the as-built drawings and listed in the engineer's certification letter.

Your project is located in the Catawba EQC District. The person who will determine the need for a final inspection, make the inspection if deemed necessary, and issue the permit to operate if all the conditions of the permit are met is Mr. Al Williams, phone #7461. The following items must be submitted to the abcVe person at least three (3) days prior to any scheduling of a final inspection and/or issuance of a permit to operate.

1. A letter from the professional engineer of record (shown on your permit) certifying that the construction ,

has been completed and is in accordance with the I approved plans and specifications. The letter must contain the following items at a minimum. ]

a. Permit number for which requesting final; I
b. A statement that the project has been constructed in accordance with approved plans and specifications. If there are any minor changes, these changes and location of the changes must be stated.

c. A statement stating that the certification is based on periodic inspections by the engineer or representative of the engineers office.
2. Copies of the bacteriological analysis with free chlorine residual recorded at the time of sample collection. The free chlorine residual must be in the range maintained by the water system. Distances on long distribution lines should not exceed 3,000 feet between sampling points.
3. Copies of the pressure / leakage test. This must include length of line, diameter of pipe tested, amount of water required to fill after test was performed and duration of the test.
4. Copies of the letter of acceptance from the utility which will operate and maintain the system.
5. Copies of record (as built) drawings if the project was not constructed exactly as permitted.
6. Copies of any information specified in a special condition of the construction permit.
7. Proof showing that any legal easement required for the 100 foot pollution free radius for wells have been obtained.
8. If the project involves chemical treatment of the water, a letter from the operator who will maintain the equipment stating his grade as well as who the back-up operator will be.
9. Copies of the Coastal Council's construction l

certification. (Coastal areas only)

10. Copies of the Water Resources Commission's approval if crossing navigable water bodies.

The following items must be submitted to the Water Supply Permitting Section, 2600 Bull Street, Columbia, SC, 29201, if the project includes a well:

1. If a test well was permitted only, the following additional information must be provided:
a. Copies of the completed well record form.
b. Copies of the geophysical / mechanical log.

c. Copies of the pumping test.
d. Copies of the required chemical analysis.
2. If a separate permit was not issued for the test well, then all the items listed in Item #1 above must be submitted with the addition of the performance curve of the well pump (s).

A permit to operate will be issued by the District EQC Office stating that the system is ready for operation once all the above requirements have been met. This letter must be in your possession before the system can be placed into operation.

This permit to operate is from DHEC only, all other permits from the City, County, or other agencies will be the owner's responsibility.

On July 1, 1993, the State Safe Drinking Water Act was amended to authorize the Department to collect an annual fee from each public water system. The monies collected are to be used for implementation of the federal Safe Drinking Water Act. The fee amount has been based on number of service connections, taps, or meters. This system will be billed annually by the Department for these fees.
