Containment Spray Additive Tank - High. Level.

This report was initiated by Incident Report 81-429.

CONDITIONS PRIOR TO OCCURRENCE: {

Mode 1 - Rx Power 99% - Unit Load 1160 MWe DESCRIPTION OF OCCURRENCE:

On October 28, 1981, at 0855 hours0.0099 days <br />0.238 hours <br />0.00141 weeks <br />3.253275e-4 months <br /> it-was noticed that the control console log reading for the Containment Spray Additive Tank level had been out of specification since 0001 hours1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

The Spray Additive Tank was declared inoperable and Action Statement 3.6.2.2 was entered at 0855 hours0.0099 days <br />0.238 hours <br />0.00141 weeks <br />3.253275e-4 months <br />.

This occurrence constituted operation in a-degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the high level was that during the filling of the Containment Spray Additive Tank, it was' filled to the.

upper limit of 4000 gallons, and minor fluctuations in the level transmitter caused the level reading to exceed specification limits.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.2 requires:

The tank was drained below the maximum level.and the. system was declared operable. Action Statement 3.6.2.2 was terminated at 1848 hours0.0214 days <br />0.513 hours <br />0.00306 weeks <br />7.03164e-4 months <br />, October 29, 1901. When the tank is filled in the future, it will not be filled to maximum.

The Control Operator was counseled for failure to detect that the tank level was out of specifications.
