LICENSE NO. UPR-23 PROMPT NOTIFICATION - PRESSURIZER _ BLOCK VALVE FAILURE

Dear Hr. O'Reilly:

In accordance with section 6.4.2 of the Technical Specifications for the H. 2;. Robinson Steaa Electric Plant. 'Jnit 2, the following event is reported.

On December 1,1981 at approximately 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br /> it was determined that the pressurizer block valves do not meet their design specifications (c-676758) -

in that they may not close against design pressure, 2485 psig. The apparent cause of failure is an undersiacd motor operator (Limitorque:

SHB-000-5), which will not develop sufficient torque to shut the block valve (Velan: 3-CM58SM). This failure could be a potentially generic (

problem and therefore it is being reported pursuant to Technical Specification 6.9.2.9.

Mr. C. Julian, NRC Region II, and Mr. B. J. Furr, Vice President - Nuclear Operations, were notified of this event on the af ternoon of December 2,1981.
