{ Attachment A to NCR QF-51 Specification C-lli-(Q), " Technical Specification for the Erection of the Reactor Building Liner Plate" contains Nondestructive Examination require-ments for the liner place under Section 9.0 which references Appendix C, Section, 1X-3300 for radiographic examination techniques.

  • Section IX-3334.3 " Number of Penetrameters To Be Used," requires that film density through the diagnostic area be from -15% to plus 30% of the density through the penetrameter which is normal to the radiation beam. However, if the density varies by more than these requirements,' an additional penetrameter is required for the exceptional area and .the density of the diagnostic area may be checked using the density of the second penetrameter. The additional penetra-meter does not need to be normal to the radiation beam.

One hundred radiographic films of liner plate welds were reviewed for compliance with the above requirement. The density check was performed on a Macbeth Quanta Log Densitometer using calibrated film strip BPC 465 (Certified on 5-15-75, due for recertification on 5-15-76) to calibrate the densitometer. It was found that fif teen films did not meet the requirements of Section IX-3334.3 of Appendix C to the specification. 1) Acceptable radiographs have been reviewed 100% for proper density. The review was conducted by two Bechtel interpreters certified.to SNT-TC-1A. -

2) A demonstration radiograph was shot which established that radiographs with I densities down to 1.54 have the sensitivity required for proper interpretation.

The radiograph was shot under the supervisi.on of MF6QCS personnel. The test radiograph provides a justified disposition for each film with an improper density. There were no films with densities less than 1.54.

3) Bechtel Proj ect Engineering has evaluated the MF6QCS review and evaluation l and concurs that the. radiographs are interpretable and, therefore, meet the requirements of the PSAR.
4) The underlying cause of the improper densities was a technique problem, probably I caused by not centering the film beneath the source. The films were accepted based on the interpreters judgement that localized areas of low density were interpretable. The following corrective action has been taken to preclude  !

recurrence: ( ,,-~s a. The NDR niihenntractor han commit ted rn more carefully centering the X-Ray.. '('- ) source; affixing a sticker to each radiograph showing the measured film densities at the penetrameters and in the veld areas; and placing a minimum of three penetrameters per each radiograph to cover the various weld buildup and backing bar thickness when an X-Ray source is used on liner plate radiographs.

b. The NDE Report prepared by Bechtel covering the interpretation of each radiograph has been changed and now requires that the measured film densities be recorded.
c. All Bechtel Level II Interpreters employed at this site will be made aware of QF-51 and this response prior to their acceptance of any radiographs.

It should be noted that all future radiographs will be accepted in accordance with specification requirements. The determination of interpretability of radiographs with low density applies only to the radiographs identified in the review necessitated by Nonconformance QF-51. G 9F5]

3) Thirty (30) films were interpreted for density by CPCo QA to verify that Bechtel's review provided accurate density readings.
4) The demonstration radiograph was reviewed. The sensitivity indic'ator (~slit) was visible on a number 7 penetrameter through which the density' measured 1.54 on a calibrated MacBeth Densitometer. The film I.D. is C-lll-ll4, Seam A, 24" South of C. .
5) The Project Engineering evaluation was reviewed. (10M telecopy, R. L. Castleberry to J. P. Connolly dated July 14, 1975)

T 6) Peabody Testins: letter from R. L. ilyard (Vice-President Operations) dated' kfd July 9,1975 was reviewed. Letter commits to more care in centering the X-ray tube over the weld and the use of three penetrameters on each shot. Also, a t'ag with film densities recorded on it will be affixed to the film.
