With reference to NCR #305 dated April 15, 1975 letter to E. F. Walder, as well as Ed Walder's informal note dated May 2,1975 confirming that there are no adverse effects resulting from the lack of nitrogen purge gas pressure for 'about 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> on April 8,1975, I wish to formally confirm that B&W is aware of this incident, but that there should be no physical damage to the generators resulting from a 2-hour loss of the minimum r: commended nitrogen gas pressure.
Wa recommend however that the gas pressure controls and monitoring procedures g
be revised to avoid reoccurrence of this incident.

A check.with the long term storage and control Engineer, Dale Martin who is
- responsible for compliance with the procedure, revealed that pressure was y
acceptable when he checked the unit early in the PM of the same date. Hartin further stated that when.he conducted his review that he noted that the l
nitrogen supply tank was low and that he had made arrangements to have t.he tank i
replaced upon his return to the fielo effice.
He further advised that the
. time-lapse _between his inspection was less than two hours as he had conducted i,. -
his inspection at 1:00 PM and the tank was replaced and the vessel re-pressurized by 2:40 PM.
Martin also stated that a positive pressure, although

lower than required by procedure, had been maintained throughout' this period of time as he had detected a discharge back flow of pressure from the steam gen-erator as the new nitrog<en tank was connected.
'It should be noted at this time that the pressure gauge being used indicates or " pegs" at a pressure reading of 2 oz. per sq. inch rather than indicating or
" pegging" at zero pressure.
Therefore, it is possible for a positive pressure to exist without a deflection of the gauge dial in.dicator.
. As a result of the above. investigation NCR No. 305 was issued, to document the y
deviation from the long term storage procedure.

Later on during erection the purge will be impossible to maintain.
.It is readily evident that some allowance must be made~, however, it may take a good deal of time to obtain concurrence on this.

- 2) Measure and record the minimum pipe wall thickness at existing pipe end utilizing' calibrated equipment.
- 3) Measure and record the' minimum pipe 0.D. adjacent to the defect.

- 4) ' Remove the defect with a medium grit aluminum oxide or silicon-carbide grinding wheel, flapper or disc.
Care shall be taken to remove' only the amount of pipe side wall to eliminate the defect. The surrounding area shall' be blended uniformerly to eliminate sharo discontinuties. 5) After eliminatio'n of the defect, examine the area by the liquid penetrant e rethod in accordance with Form 84 (attachment to G-27Q) for 2" and under piping, class CCB and paragraph NB-2500 of the Nuclear Power Plant Com-I par.enets ponents code for the applicable product form. __, 6) Afterthedefecthasbeenremovedortheindicabonreducedtoanacceptablesize,measureandrecordthe minimum pipe 0.0. at the point of grinding. i 7) Deduct the_ amount removeLin_ grinding ff from the minimum measured _ttall thigkagsL e.lermined in Slep_2_ dove. d If the remaining pipe side wall is equal to or greater than 87.5 percent of the nominal wall thickness, the recai shall be complete and accepted. The nameplates shall be as shown on Drawing 7220-E-49 except that the thickness shall be 1/16 inch." Contrary to thq_ab_ove. lamicoid nameplates have not been provided for the 34 circuit breaker devices. 2b. Specific _ation 7220-C-1 paragraph 5.0 states in,psrt: "Each piece of equipment shall be identified witi-a securely fastened metal tag, bearing the purchase _ _ order number and the appropriate equipment number as specified. Contrary to the above, metal tags were not provided with the 34 circuit breakers. 3. Specification 7220-E-205 paragraph 12.1 states in part: "'Ihe method of preparation for shipment shall adequately protect the switchgear and all accessories against corrosion. dampness, breakage or vibration iniury that might be encountered in transp_o_rtation and_ handling". Conrrary to tin. above two_(2) circuit breakers (Serial No. F-8-3-8-B and L-8-8-8-C) sustained damaze around the area of the hinges of the arc chutes (see attached photos). Apparent damage was caused by circuit breakers shif ting to the rear on the shipping skid. NOTE: Wooden block on the front of skid is bolted down, while block on the rear of skid is only nailed down. The rear wooden block gave uay in each case. 4.
