'4.- Occurrence Date: 25 April 1978

5. Identification of Occurrence:

7ecay Heat Removal Pump 1-A inoperable creating a condition contrary to Technical Specification 6.9.1.8.1.

6. Conditions Prior to Occurrence:

l Mode 6 refueling.

7. Description of Occurrence:

At 2200 on 25 April 1978 it. was observed that low flow condition existed on Decay Heat Removal Pump 1-A (DHP l-A). Upon disassembly of DEP l-A it was discovered that the pump shaft had sheared at the impeller end.

The pump manufacturer and supplier representatives have been oc site and pump shaft breakage evaluations are now in progress.

The cause-of this event is presently being evaluated. Upon determina-

tion of cause a revision to th*is report will be submitted.

9. Analysis of Occurrence:

No hazard to the health and safety of the plant or general public as plant was defueled.

10. Corrective Action:

Corrective actions are presently being formulated by the pump manufact-urer and supplier.

11. Failure Data:

This is the second occurrence of an event of this type.
