At that time it was detennined the plant was operating outside its design basis, N= iuse both of the Borated Water Storage Tank Invel Transmitters which prwide signals to the control room were not seismically qualified.

Unqualilled transmitters had been puzchased and installed because the plant safety listing, which was used to deterinine the purchasing specifications, incorrectly identified the safety function of the transmitters as pressure retention only.

The transmitters have been replaced with qualified transmitters. The safety listing is being reviewed to ensure that similar problems do not exist.

violations identified in NRC jnspection 89-200, contract engineers were '

EVENP ANALYSIS

'Ihe Dorated Water Storage Tank is the main source of water for High Pressure Injection (BQ), Irw Pressure Injection (BP) and Reactor Ik111 ding Spray (BE).

During an accident operators use the INST level to detemine khen to charge the ,

suction for these systems frce the INST to the reactor building sunp (BP,TK). '

only the seismic qualification of the transmitters was inadequate. 'Iberefore, the only time they would be inoperable is following an earthquake. Other INST  :

level indicators, which are seismically qualified, provide signals to the remote shutdown panel. Operators could have used these instruments to monitor BET level following an earthquake if the control room instruments lem inoperable. Because there has been no significant earthquals activity in the plant area, and because the probability of earthquakes in this area is 1cw, there was no reduction in plant safety due to this event.

t CAUSE

'Ihe subject trans.nitters were purchased in 1987. 'Ihe personnel who purchased the transmitters did not specify seismic qualification because the plant safety '

i listing incorrectly identified the safety function of the transmitters as pressure rutention only. In fact the transmitters must moet the requirements of Regulatory Guide 1.97, which include seismic qualification.

The unqualified transmitters have been replaced with qualified transmitters.

'Ihe safety listing has been Invised to indicate that the subject transmitters ,

must meet the requirements of Reg. Guide 1.97  ;

'Ibe safety listing will be reviewed to ensure that all Reg. Guide 1.97 relatnd instruments are properly identified.

The safety listing has recently became a part of the configuration management  ;

process. Procedures for this process will ensure that the safety listing is properly mintained in the future. '

PREVIOUS SIMIIAR EVDTIS As stated in IIR 89-35, there have been other events related to operation outside the design basis at Crystal River Unit 3. 'Ibese are listed in IIR 89-

33. None of these events involved inadoquacies in the safety listing.
