Contrary to. Environmental Technical Specification 3.2.2,-the weekly I-131 charcoal cartridge from Environmental Sample Station C-41 was not analyzed.

Description of.0ccurrence:

The plant was notified on 3 June 1980 by the analyst that on 23 May 1980, it was discovered that the weekly I-131 charcoal cartridge for the week ending 1 April 1 1980 was inadvertently discarded. A review cf this event determined this cartridge to be~the only one missing and all others showed no detectable activity.

Designation of Apparent Cause:

The cause of this event is attributed to non-FPC personnel error occurring during the preparation of the charcoal cartridge for analysis. It was discovered that the charcoal cartridge was inadvertently-discarded.

Analysis of Occurrence:

This event did'not result from the operation of Crystal River

. Unit #3, nor is the operation of the plant affected by it.

Corrective Action: - Personnel responsible were admonished to exercise strict control of samples during collection and preparation.

~ Failure Data: This is the first occurrence of this type reported.
