CONFIRMATION OF TELECON REPORT TO R. H. WESSMAN OF YOUR OFFICE AT 1815 on 11 MAY 1978

1. Report No.:

50-302/78-015/047-0

2. 7'acility: CrystalRiverU$it#3
3. Report Date: 12 May 1978 4-4 Occurrence Date: 11 May 1978
5. Identification of occurrence:

Spill of radioactive liquid and is reported in accordance with 10 CTR 20. Part 403.

6. Conditions Prior to occurrence: i

~

Mode 6 refueling (defue1ed) i i

7. Description of occurrence:

1 1

At 1515 on 11 May 1978 while transferring spent resin from the spent resin storage tank to the solidification cask on the vendor transport vehicle, a Tuptured transfer line on the transport vehicle caused a release of less than two gallons of transfer water to a storm grating.

Using the gamma isotopic analysis of the transfer water,

\ the58.

Co total amount of radioactivity which entered the drain was 20 microcuries primarily The immediate action was to secure the resin transfer, spread " safety set" l to ermrnia rbe 14-rid, p* gnif a .f hdctc c:ca as a "1 1

6. z.caauou area' . Inere was no measurable activity at the outfall of the stora drain and all spilled resin has been recovered.
8. Designation of apparent Cause:

This event was due to the rupture cf a transfer pipe on the vendor transport vehicle.

9. Anslysis of Occurrence:

There was no safety hazard to the plant or public as the spill was immediately contained and proper radiation protection measures were initiated.

10. Corrective Action:

Corrective actions to preclude recurrence are being formulated at this time. Upon resolution, this report will be revised.

L1. Tailure Data:

This is the first occurren<:e of an event of this type.
