5. Identification of occurrence:

Spill of radioactive liquid and is reported in accordance with 10 CTR 20, Part 403.

6. Conditions Prior to Occurrence:

1 Mode 6 refueling (defueled). J

7. Descriptien of Occurrence:

At 1515 on 11 May 1978 while transferring spent resin frem the spent resin storage tank to the solidification cask on the vendor transport vehicle, a ruptured transfer .

line on the transport vehicle caused a release of less than two gallons of transfer water to a storm grating. Using the ga:mna isotopic analysis of the transfer water, f, the total amount of radioactivity which entered the drain was 20 microcuries pri=arily Co 58. The immediate action was to secure the resin transfer, spread " safety set" to contain the liquid, post guard and isolate area as a "high radiation area". There was no seasurable activ1cy at the outfall of the stors drain and all spilled resin O

has been recovered.

6. Designation of Appatent Cause:

This event was due to the rupture of a transfer pipe on the vendor transport vehicle.

9. Analysis of Occurrence:

'here was no safety harard to the plant or public as the spill was immediately centained and proper radiation protection reasures were initiated.

10. Corrective Action:

To prevent unwanted fluids frem entering the storm grating, a Request for Engineering Information (REI) is being fomulated to enclose the area where the vendors equipment is located with a permanently installed retaining barrier, aoout 6" high. Included in the enclosed area would be a sump where spilled fluids would collect for proper disposal. Corrective actions to preclude recurrence have been taken by the vendor.

11. Failure Data:

This is the first occurrence of an event of this type.
