Facility: Crystal River Unit #3

3. Report Date: 13 March 1978

4. Occurrence Date: 5 March 1978

5. Identification of Occurrence:

Unplanned radioactive gas release to the atmosphere reported in accordance with 10 CFR 20.403.

6. Conditions Prior to Occurrence:

Mode 5 cold shutdown operation.

7. Description of Occurrence:

At 1410, an unplanned gas release occurred during plant cooldown. While desassing -

Reactor Coolant System, in preparation for RCS draindown, the Wasta Gas Surge tank  ;

was isolated by a temporary change to the Hydrogen Addition and Degasification "

Procedure (OP-205) . The purpose of the change was to put all the high activity gasses into one waste gas decay tank. After completion of degasification, the waste gas header was over-pressurized blowing the Auxiliary Building loop seals.-

A total of 128 curies of noble gasses, primarily Ze-133, was released. The instantan-g acus release rate was 5% .of the most restrictive noble gas limit set forth in Environ-mental Technical Specification.2.4.2.A(1). . .

8. Designation of Apparent Cause: -

The cause of this event was personnel error, in that the' camporary procedure change to OP-205 was not followed and the gas header was over-pressurized, blowing the Aux 111nry Building loop seals. l l

9. Analysis of Occurrence:

The health and safaty of the public was not affected in that the instantaneous release rate was 5% of the most restrictive noble gas limit.

10. Corrective Action:

Supervision & concerned personnel have been made aware of the consequences of not following approved procedures both temporary and per:anent. Discrepancy Reper

11. Failure Data:

This is the 7th occurrence of this event due to blown loop seals.
