, SUPPLEMT.NTARY INFORMATION Report No.: 50-302/79-019/03L-0 Facility: Crystal River Unit #3 Report Date: 15 March 1979 Occurrence Date: 23 February 1979 Identification of Occurrence:

One remote shutdown instrumentation channel inoperable contrary to Technical Specification 3.3.3.5.

Conditions Prior to Occurrence:

Mode 3 operation (hot standby)

Description of Occurrence:

While conducting routine plant surveillance, it was discovered that the motor driven emergency feedwater pump discharge pressure gauge EF-2-PI, was out of calibration. Further investigation revealed that the dampening snubber located in the sensing line to the gauge had failed. The snubber was eplaced. The indicator was calibrated, tested, and returned to service 3/12/79.

Designation of Apparent Cause:

The cause of this event is attributed to an inoperable dampening ,

snubber located in the sensing line to the indicator.

Analysis of Occurrence:

No hazard to the plant or general public as the plant was in a safe shutdown condition.

Corrective Action:

The dampening snubber was replaced. The indicator was calibrated in accordanca with SP-169 and tested per SP-349, " Emergency Feedwater System Operability Demonstration." The indicator was returned to service 3/12/79. No further corrective action required.

First occurrence reported for EF-2-PI.

Failure Data:

First occurrence reported for EF-2-PI.
