One re=ote shutdown instrumentation channel inoperable contrary to Technical Specification 3.3.3.5.

Conditions Prior to Occurrence:

Mode 1 power operation (1007.)

Description of Occurrence:

At 0230, while conducting emergency feedwater system operability demonstration, SP-349, it was discovered that the motor driven e=ergency feedwater pump discharge pressure gauge EF-2-PI was inoperable. The gauge indicated approximately 1200 psig with the pump secured. Further investigation revealed that the damage to the gauge probably resulted from excessive vibration and tirges from ete pump starting. Tha gauge was replaced, calibrated, tested, and returned :o service 3/22/79.

Designation of Appar Cause:

The apparetit cause c f the pressure gauge failure is attributed to excessive vibration and surges resulting from emergency feedwater pump starts.

Analysis of Occurrence:

No effect on public health and safety as the plant was in a steady state condition and emergency feedsater flow from the pu=p would not have been inhibited if it were required.

Corrective Action:

The gauge was replaced with a glycerin-filled gauge to reduce vibration and starting surges. The instrument was calibrated, functionally tested, and returned to service at 0421 on 22 March 1979.

Failure Data:

This is the second occurrence reported for EF-2-PI. (Reference LER 79-19)
