. SUPPLEMENTARY INFORMATION

3. Report Date: 15 January 1979
4. Occurrence Date: 24 December 1978
5. Identification of Occurrence:

Inoperability of two of three reactor coolant systems leakage detection systems contrary to Technical Specification 3.4.6.1.

6. Conditions Prior to Occurrence:

Mode 1 power operation (99%).

7. Description of Occurrence:

At 1300 the sample pump for Radiation Monitor RMA-6 failed, rendering the Conta.1-ment atmospheric Iodine and gaseous radioactivity monitoring system inoperable.

The six hour action statement was entered and Work Request 4258 was initiated.

The sample pump was removed and replaced with a spare pump. Radiation Monitor RMA-6 was returned to service and declared operable at 1500, ending the six hour actica statement.

8. Designation of Apparent Cause:

The cause of this event is attributed to the failure of the RMA-6 sample pump.

The carbon vanes had worn and distorted causing the pump to overheat and trip.

9. Analysis of Occurrence:

There was no safety hazard to the plant or general public as redundant monitor-ing system was available.

10. Corrective Action:

No corrective action, as normal maintenance restored operability of the pump.

An engineering study is underway to evaluate RMA-6 pump failures. Results of this evaluation will determine any additional corrective action.

11. Failure Data: -

Four previously reported occurrences, but only one previously reported RMA-6 pump failure.
