, SUPPLEMENTARY INFORMATION Report No.: 50-302/79-095/03L-0 Facility: Crystal River Unit #3 Report Date: 22 October 1979 Occurrence Date: 30 September 1979 Identification of Occurrence:

Remote shutdown indicator for "B" decay heat cooler outlet temperature DH-40-TI inoperable, contrary to Technical Specification 3.3.3.5.

Conditions Prior to Occurrence:

Mode 1 power operation (74%)

Description of Occurrence:

At 1400 during the performance of SP-300, Operating Daily Surveillance, it was discovered that DH-40-TI was providing erroneous indication. DH-40-TI indicated approximately 240*F in contrast to actual system tempe.ature of approximately 105*F.

Maintenance restored operability and DH-40-TI was returned to service following satisfactory completion of SP-161, Remote Shutdown Instrumentation Calibration on 4 October 1979.

Designation of Apparent Cause:

The cause of this event is attributed to a defective meter movement in the indicator for DH-40-TI.

Analysis of Occurrence:

No effect upon the plant or general public.

Corrective Action:

The defective meter movement was replaced with a spare and was returned to service.

Failure Data:

This is the first occurrence of this type reported.
