. . . SUPPLEMENTARY INFORMATION Report No.: 50-302/79-094/03L-0 Facility: Crystal River Unit #3 Report Date: 19 October 1979 Occurrence Date: 27 September 1979 Identification of Occurrence:

Failure to perform post maintenance testing of "B" OTSG blowdown valve MSV-148 contrary to Technical Specification 4.6.3.1.

Conditions Pri< to Occurrence:

Mode 1 power operation 100%.

Description of Occurrence:

At 1309 it was reported that "B" OTSG blowdown isolation valve MSV-148 was not properly tested prior to being returned to ser-vice, following maintenance conducted on 2 July 1979. MSV-148 was returned to service at 1105 on 28 September 1979 following completion of appropriate past maintenance testing.

Designation of Apparent Cause:

The cause of this event is attributed to defective procedures.

Surveillance Procedures did not contain provisions for testing MSV-148.

Analysis of Occurrence:

No effect upon the plant or general public. MSV-146 was placed in the closed position.

Corrective Action:

A review of Surveillance Pro'_eiures has been conducted to de-termine generic implicatieas. toplicable procedures will be revised to include provisions for testing Technical Specifica-tion-related valves.

Failure Data:

This is the first occurrence of this type reported for MSV-148.
