Boric Acid Storage Tank Volume Less than allowed by Technical Specifications.

CONDITIONS PRIOR TO OCCURRENCE:

12/30/77 - Operational Mode 1, Reactor Power: 30%

1/04/78 - Operational Mode 1, Reactor Power: 54%

DESCRIPTION OF OCCURRENCE:

At 2151 hours0.0249 days <br />0.598 hours <br />0.00356 weeks <br />8.184555e-4 months <br />, on 12/29/77, the reactor was taken critical following a forced outage. At 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br />, on 12/30/77, the Control Operator observed that the boric acid storage tanks were below the required 5106 gallon level. Technical Specification Action Statement 3.1.2.8.a was implemented. Shift personnel had anticipated the need for additional boric acid and had begun batching on the previous 4xl2 shift .. Boron addition to the Reactor Coolant System as a result of Xenon decay and burnout was greater than the batching could supply.

At 1255 hours0.0145 days <br />0.349 hours <br />0.00208 weeks <br />4.775275e-4 months <br />, after multiple boric acid batches had been trans-ferred to the storage tanks the level was sufficient to terminate Action Statement 3.1.2.8.a. Boron addition to the RCS continued

  • and at 1525 hours0.0177 days <br />0.424 hours <br />0.00252 weeks <br />5.802625e-4 months <br />, Action Statement 3.1.2.8.a was again implemented with storage tank level less than 5106 gallons. Batching continued until at 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br /> the level was greater than 5106 gallons and Action Statement 3.1.2.8.a was terminated.

On 1/4/78 at 1921 hours0.0222 days <br />0.534 hours <br />0.00318 weeks <br />7.309405e-4 months <br />, the boric acid storage tanks were again less than 5106 gallons. The unit wa~ recovering from a reactor trip that had occurred at 0225 hours0.0026 days <br />0.0625 hours <br />3.720238e-4 weeks <br />8.56125e-5 months <br />. With the unit at 54%

power, Xenon burnout and decay required boration which reduced the.storage tank levels to less than required by Technical Speci-fication 3.1.2.8. Action Statement 3.1.2.8.a was implemented and batching continued until at 0225 hours0.0026 days <br />0.0625 hours <br />3.720238e-4 weeks <br />8.56125e-5 months <br /> on 1/5/78, the tank levels were above 5106 gallons and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

These occurrences were a result of reactor startups. Xenon decay and Xenon burnup necessitated boration rather than the normal dilution. The resulting boration caused the volume to decrease to

less than the 5106 gallons allowed by Technical Specification 3.1.2.8.

These occurrences were caused by a procedural inadequacy in that the log used to maintain *tank levels only required 70% or 5106 gallons to be maintained. Although the storage tank levels were normally maintained above this level, little margin existed for operational considerations.

ANALYSIS OF OCCURRENCE:

Technical Specification Action Statement "a" for :t.co 3.1.2.8 states that with the boric acid storage system inoperable, restore the storage system to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at* least Hot Standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> anc;1 borated to a shutdow:n margin equivalent to 1% ~K/K at 200°F; restore the boric acid storage system to operable status within the next_7 days or be in Cold Shutdown

The boric acid storage tanks volume was increased to greater than 5106 gallons within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, therefore, no mode reduction was required.
  • CORRECTIVE ACTION:

Batching of boric acid and transfers to the storage tanks brought the levels back to within the Technical Specification limits.

To prevent reoccurrence of this type of event, an administrative limit of 92% has been established. This limit will allow for oper-ational considerations while maintaining the storage tanks well within the Technical Specification limits.
