DNB Parameter Outside Technical Specification Limits (Pressurizer Pressure)

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1, Reactor Power 90%

DESCRIPTION OF OCCURRENCE:

At 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br />, pressurizer pressure decreased to 2150 psig. Technical Specification Action Statement 3.2.5 was implemented by the Shift Super-visor. At 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />, valve 1PS3 (Pressurizer Spray Valve) was isolated.

The pressurizer pressure returned to normal and the action statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A pneumatic valve operator adjustment problem caused 1PS3 to remain off of its closed seat. Continuous spray flow through the valve caused the pressurizer pressure to decrease below the 2220 psia allowed by Technical Specification 3.2.5.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.2.5 states that with any DNB parameter ex-ceeding its limit, restore the parameter to within its limit within two (2) hours or reduce thermal power to less than 5% of rated thermal power within the next four (4) hours. The pressurizer pressure was restored to within its DNB limit within the time allowed by the Techni-cal Specification, therefore, the LCO was not violated. At no time was there any danger to the health and safety of the general public or site personnel.

CORRECTIVE ACTION:

The "zero" position (fully closed) on the pneumatic valve operator was reset. The valve was restroked with satisfactory results. The valve leakage terminated. and valve 1PS3 was returned to service.
