~ Occurrence Date: 4/23/81 Facility: Salem Generating Station, Unit 2 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

Failure of No. 22 Auxiliary Feedwater Pump to Start CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

While attempting to start the No. 22 Auxiliary Feedwater Pump, it was discovered that the relay flags for phases A and C on Breaker 2BlD (the pump's feeder breaker) had dropped. Action Statement 3.7.1.2 was entered at 0200 hours0.00231 days <br />0.0556 hours <br />3.306878e-4 weeks <br />7.61e-5 months <br />. Investigation revealed that the breaker would not latch closed.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure - The wipe of the breaker's trip latch had drifted out of adjustment.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.1.2 requires that with one Auxiliary Feedwater Pump inoperable, restore at least three Auxiliary Feed-water Pumps (two capable of being powered from separate vital busses and one capable of being powered by an operable steam supply system) to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION:

In accordance with Maintenance Procedure M3D, the wipe of the trip latch was reset to the correct travel. Megger checks of all input phases wer~ made and found to be satisfactory. The breaker was tested satisfactory and the Action Statement terminated at 1100 hours0.0127 days <br />0.306 hours <br />0.00182 weeks <br />4.1855e-4 months <br /> on April 23, 1981.
