  • IP 11 A. PURPOSE

'The purpose of this installation procedure is to provide direction for the inspection of jumpers and control wiring, installation of replacement wires where necessary, and the installation of tee drains where_necessary in safety-related Limitorque motor valve operators. The inspection /installa-tion performed under this procedure will resolve concerns generated by NRC IE Notice 86-03.

  • IP 11 C.2 Quality Control C.2.1 Performs inspections in accordance with the requirements of this procedure.

D. DEFINITIONS AND ABBREVIATIONS D.1 "QC " in the left margin, in front of a step denotes a QC Tnspection point. That step requires one or more QC inspection functions as denoted by the last initial (C, E, W, V, or Hold). QC inspection functions are defined in AP 1.7 (reference 2.5). (The responsibilities of the installing personnel and QC inspector are defined in Section I of this procedure.)

D . 2. A "QC HOLD" in the left margin denotes a point beyond which work shall not proceed without the consent of Quality Control. This consent shall be indicated by completion of QC activities required by the hold point and QC sign-off of the documentation associated with these activities.

in exactly the same configuration as the one previously removed. Verify per drawings listed in Appendix I.

E.2 Ensure that all terminals are de-energized prior to beginning inspection on Limitorque actuators.

E.3 Distribution of controlled construction documents shall be in accordance with EDP-8.

E.4 Protective tagging shall be obtained in accordance with WACP 10.1.2.

E.5 Work required by this installation procedure shall be performed in accordance with WACP 10.1.1. One WRED will be issued for the entire work scope and a separate WTF for each valve actuator to be inspected / upgraded.

! Additional WTF's may be issued to control and document testing per Section I of this procedure.

  • IP 11 G. PREREQUISITES QCW G.1 Prior to beginning work, the installation personnel shall review this procedure in its entirety, become familiar with its contents and have issued to them a controlled set of installation documents. A pre-planning meeting shall be held with the RE, Electric-ians, ALARA, and QC present.

G.3 The Shift Supervisor and QC shall be notified in advance that this procedure is to be performed and shall receive a controlled copy of this procedure.

Documentation established on applicable WTF's.

H. PROCEDURE H.1 Remove limit switch compartment cover and check that all terminals are' de-energized using a volt meter or a rpulti-me ter.

H.2 For each valve operator listed in Appendix I, perform the tasks identified below hnd document, via the appropriate sign offs, on the Appendix II checklist.

There is a separate checklist for each valve operator.

H.3 Inspect control wires and jumpers. Use the following I criteria to determine acceptability of existing control wires by visual inspection.

H.3.1 The wire is acceptable if it can be positive-ly identified as one of the following quali-fied wire types by markings on the wire insulation:

Raychem Flamtrol SIS H.3.2 For wires which do not carry any wire identi-fication marking, identification shall be established, beyond a reasonable doubt, if the existing jumper wire (s) are one of the two qualified wire types by a similarity comparison which utilizes, but is not limited to the following inputs:
a. Wire insulation color and texture similar to one of the two qualified wire types which has been positively identi-fied by wire marking in the same opera-tor,
b. Wire size consistent with one of the two qualified wire types in the same operator.
c. Terminal connectors are consistent with those used for qualified jumper wires in the same operator.

H.4 If any internal control wire or jumper cannot be positively identified as a qualified wire type using the acceptance criteria in Section H.3.1 and H.3.2, replace the unidentifiable wires with Rockbestos Firewall SIS #12 or #14 AWG in accordance with the following guidelines:

QCV H.4.1 Replacement wires shall be fabricated in accordance with IS-E-07.

H.4.2 Replacement of any internal control wire (s) shall be performed one at a time.

H.4.3 With existing wire terminations intact, cut the wire in half.

H.4.4 Remove one end of existing wire. Remove cut piece and install one end of the replacement wire at the terminal point, then remove the Rev. No. 0 ,

H.8 For relatively newer actuators whose design includes l provisions for grease expansion plugs, ensure that the shipping cap has been removed to allow the expansion plug to function properly.

  • IP #1 J.2 For each actuator which did require internal jumper wire replacement, request the Maintenance Department to s perform post-maintenance stroke testing per Maintenance Procedure MP 59.3, Section 8.0, Steps 8.1, 8.3, 8.4, 8.5, 8.7, 8.8, 8.9, 8.10 (if brake installed), 8.12, and 8.13. A separate Work Tracking Form (WTF) will be issued by the Responsible Engineer to the Maintenance Department to authorize and document this testing.

Also, the WTF will include required operations surveil-lance testing which may be required to be performed.

K. SIGN-OFF COMPLETION K.1 All steps in Section H have been completed as required by the procedure for each operator on the Appendix I list.

r m ENCLOSURE 2 Result Summary of Limitorque Control Wire Inspections Performed to Address NRC IE Notice 86-03 Installation Procedure M1-86-024, IP fl delineated inspection requirements for all 12 Limitorque valve actuators located inside containment and requiring environmental qualification. One Limitorque valve actuator located in the Main Steam Tunnel and 4 Limitorque valve actuators located in the Reactor Building drywell entrance were also inspected at this time, since they are difficult to access during normal plant operation.

The objective of this inspection was to identify any Limitorque valve actuator control wires or jumpers for which environmental qualification has not been established.

All control wires that could not be positively identified by wire markings, applying the acceptance criteria in Procedure M1 024, IPfl, Step H.3, were replaced with environmentally qualified wires.. The replacement wire used was Rockbestos Firewall III SIS. This very conservative approach resulted in the replacement of many more wires than actually required to assure qualifica-tion. For example if an actuator had Raychem Flamtrol SIS Wiring (qualified for containment), but some of the wires had no marking due to.their short length, then all of the wires were replaced.

As a-result of the inspections performed on the Limitorque valve actuators, wires were replaced in 12 actuators. The wire types replaced include:

- Raychem Flamtrol SIS

- G.E. Vulkene SIS

- Single Conductors from Multi-Conductor Control Cables (These jumpers are judged to be cross-linked polyethelene insulated singles from qualified multi-conductor control cables)

- Unidentifiable Types (SIS wires identical to adjacent wire but without markings)

Although not necessarily deficient, many internal wires were-replaced to eliminate any doubt as to their qualification. As a result of the inspections and wire replacements performed, it is established that, at this time, the control wire type used in all 17 actuators consist solely of qualified Rockbestos Firewall III SIS. Full environmental qualification documentation for Rock-bestos Firewall III SIS wire is available in the Equipment Qualification Reference File at the FitzPatrick Plant.

ENCLOSURE 3 Schedule-for Inspection of Outside Containment Actuators

'1. The Limitorque actuators outside containment within the scope of 10 CIT 50.49 will be inspected. The object of the inspection is to; confirm the presence of qualified internal wires and, jumpers. The inspection will.be performed in conjunction with the FitzPatrick plant Preventative Main- 1 3tenance Program activities for. the subject Limitorque actuators. According to.this schedule inspections will be

-completed by the end of the maintenance outage schedule for September;or October 1986.

2. This schedule is justified based on several considerations:
a. The results of the inside containment inspection-conducted in March 1986 (Enclosure 2) indicated a high probability that the Limitorque wiring outside contain-ment is of.a of qualified type.with documentation available in the FitzPatrick Equipment-Qualification (EQ) File.
b. Since postulated accident environments outside contain-ment are less severe than those inside, it is highly probable _that the wiring is qualified-(including wiring insulation such as PVC). Qualification reports for PVC-insulated wire in Reactor Building environments are on file in the FitzPatrick plant EQ File, including a specific test report on a Limitorque with this type of.

wiring insulation (steam environment test).

c. Because of the small and often intermittent values of electrical' currents in these wires, Limitorque control wiring. insulation is not subject to significant aging
due.to internal heating affects or significant elec-trical stress. Therefore, any thermal age-related
degradation, which may be experienced outside contain-ment, would be restricted to local ambient temperature-effects. Since normal temperatures in these' areas are 80'-90*F, insignificant degradation would be expected for most insulation types under normal conditions,
d. The continuous wiring ratings (UL) of jumaer wires which could potentially be installed in the subject outside containment actuators are greater in most cases then the peak temperatures postulated for.the outside-containment high energy line breaks (HELB). This provides a high degree of confidence that the wires would remain functional during postulated events which may cause thermal stresses.

e. Postulated radiation dose parameters-for the majority of outside containment valve actuators are less than the published radiation damage threshold values.for the types of insulation which could conceivably be in-stalled.(i.e., PVC, XLPE). Thus, insulation integrity would be maintained following postulated events result-ing in a significant radiation dose.
f. For outside containment actuators, significant radia-tion stress occurs after postulated LOCA events which occur inside containment. Significant thermal stresses for the outside containment actuators occur following outside containment HELB events. Since significant thermal and radiation stresses do not occur simultane-ously, there is no significant radiation-thermal aging synergistic effects for outside containment actuators,
g. In preparing the schedule for the outside containment wiring inspections, the actuators to be inspected have been grouped into four categories based on the severity of postulated accident environments. The postulated environments will be considered in scheduling these inspections.
h. Procurement of wiring that could be used as field-l, installed jumpers in Limitorque internals has been limited to a few types since plant construction.

i. A significant portion of the outside containment valve o3erators included on the JAF EQ List are normally in their post-accident position. Some of the remaining valve actuators are immediately repositioned by auto-matic Loss of Coolant Accident or High Energy Line Break signals and do not require further repositioning.
