  • PROC. NO: PS0 #28 5.0 RESPONSIBILITIES 5.1 Department Superintendents, subject to Superintendent of Power review, are responsible for the final deter-mination of applicability and the subsequent trans-lation of recommendations into plant actions.

The Department Superintendents shall initiate procedure changes, operating instructions, training, preparation of NRC response letters, etc., as required.

5.2 The Manager of Operational Evaluation shall audit this procedure, thereby initiating a routine evaluation to review the program effectiveness.

5.3 The PPRS is responsible for:

a)

Initial review of operating experience for applicability.

b)

Assignment of individuals for operating experience review.

c)

Tracking operating experience until final action completion.

A hard copy file (maintained by a member of the PPRS staff) shall contain the following:

1.

Ccpy of the operating experience document.

2.

Copy of correspondence which details evalua-tions and rec,mmendations (DER form).

3.

Copy of correspondence which delineates actions taken on recommendations (GER form).

4.

Copy of correspondence sent off site to report action taken, if applicable (example:

SILs).

5.

Final "close out" document (DER form).

5.4 The Assistant to the Resident Manager for licensing is responsible for ensuring that action dates specified on the OER form are compatible with the requirements for

  • PROC. NO: PSD #28 6.0 SPECIAL INSTRUCTIONS / PRECAUTIONS 6.1 It is crucial that any action commitment (procedure change, training, maintenance, modification, etc.) made to an outside agency, be fully understood by all affected supervisory personnel prior to finalizing a commitment.

"6. 2 To prevent contradictions, training initiated to

implement lessons learned from operating experience review shall normally be provided to plant personnel via the Training Superintendent after approval by the Superintendent of Power.

6.3 Significant Operating Experience Reports (SOER) prepared by INPO involved a great deal of analysis and integration of industry recommendations.

As a matter of policy, INPD SOER recommendations will be considered for implementation without modification.

If plant conditions are such that the recommendations cannot by physically implemented, alternate actions will be implemented after approval of the Resident Manger.

6.4 All experience reports should be reviewed from a generic viewpoint (e.g., valve problems at a PWR should not be declared not applicable unless the experience is reviewed to see that similar p roblems could not occur at JAFNPP).

i 7.0 PROCEDURE 7.1 Generation, Receipt and Routing of Operating Experience Documents 7.1.1 Internal (JAF) generated operating experience documents such as Occurrence Reports (OR) are initiated by Department Superintendents or the Shift Supervisor, in accordance with Reference 3.4 (AP 8.2).

The Shift Technical Advisor (STA) also logs receipt of the document into the operating experience review system and thus initiates the piocess of operating experience review and notification of other-interested industry groups. Detailed [

In addition, figure 8.1 illustrates a general flow path of the entire operating experience review process at JAFNPP.

7.1.2 NRC, industry, White Plains Office (Nuclear Generation) and certain INPD generated operating experience documents are routinely received by the Resident Manager's Office as part of the plant mail as described in Reference 3.2 (WACP 10.1.9).

These documents are listed in section 4.1 and are processed as indicated in section 7.2 and 7.3 of this procedure.

In addition, Figure '8.1 11-lustrates a general flow path of the entire 4

operating experience review process at JAFNPP.

7.1.3 INPO Significant Event Reports (SER) are j

received by the PPRS on NOTEPAD.

The PPRS enters the SER in the Operating Experience Status Log and initiates the review process as indicated in section 7.2 and 7.3 of this procedure.

A general flow path of the entire operating experience process at JAFNPP is illustrated in Figure 8.1.

i 7.2 Preliminary Review and Assignment of Detailed Re-view / Evaluation of Operating Experience Documents 7.2.1 JAFNPP Internally Initiated Operating Exper-ience.

7.2.1.1 The On-Shift STA (or other individual assigned by PPRS during plant outage periods) logs the OR into the Operating 4

Experience Status Log (Exhibit 9.1) following initiation, preliminary classification and critique (if any) by the Shift Supervisor / Department Super-intendent and performs a review /evalua-tion of the OR. (Exhibit 9.2)

The root cause of the event which resulted in initiation of the OR if the root cause can be readily determined at this point in time, d.

Reportability as required by NUREG 1022, Licensee Event Report System through Supplement 1.

7.2.1.2 The STA comment / evaluation is attached to the OR and forwarded to the Operation Superintendent for his review in accordance with AP 8.2, and a copy of the STA comments are forwarded to the PPRS.

NOTE:

No additional action (review, evaluation, recommendation, or other comment) is required of the PPRS for OR's unless as part of the PORC Review of the OR and subsequent LER (if any) addition action is specifical-ly assigned.

  • PROC. NO: PSO #28 7.2.2.1 Operating experience documents received by the Resident Manager's Office are reviewed by the Resident Manager as part of the daily mail and are routinely assigned to the PPRS.

Those Operating Experience Documents which the Resident Manager determines are significant and require immediate evaluation may be directly assigned to a

respodsible individual (RI).

A copy is also provided to the PPRS.

7.2.2.1.1 the Resident Manager's Office also opens an ARMS action item cor-responding to the Responsible Individual (PPRS or other RI) assigned in paragraph 7.2.2.1 above.

7.2.3 INPO initiated Operating Experience (NETWORK) 7.2.3.1 Significant Event Reports (SER) received on NETWORK are entered in the Operating l

Experience Status Log by the PPRS and then reviewed and assigned as indicated in paragraph 7.3.

7.3 Detailed Review, Evaluation and Recommendations 7.3.1 The PPRS enters those operating experience documents that have not yet been entered in the Operating Experience Status Log (Exhibit 9.1) and reviews the document for applic-ability to JAFNPP.

Documents determined to be not applicable (NA) are so noted on the Operating Experience Review (GER) form and the form is routed to the Superintendent of Power for approval (paragraph 7.3.7).

7.3.2 For those documents which are applicable and have not already been assigned to a Respon-sible Individual (RI) by the Resident Manager the PPRS assigns an individual as the Responsible Individual and assigns a due date to the task.'

NOTE: Responsible Individual assignment will normally be made to personnel within the Performance and Reliability, staff,

however, some operating experience documents may (due to complexity or the nature of the subject matter of the document) require assignment to a

department superintendent or others outside of the Performance and Relie-bility staff.

7.3.3 The assigned Responsible Individual reviews the Operating Experience Document, evaluates its impact and makes a recommendation with respect to disposition.

Those documents which are determined to be not applicable (NA) to JAFNPP are so noted on the OER form and the form is routed to the Superintendent of Power for approval (paragraph 7.3.7).

7.3.4 For those documents which are applicable to JAFNPP, the recommendation is forwarded to the appropriate Department Superintendent (s) for review / evaluation of the recommendations.

If the Department Superintendent is in agreement with the recommendations of the RI, the document is forwarded to the Super-Intendent of Power for approval.

7.3.5 When the Department Superintendent is in disagreement with the recommendation of the RI, then an attempt to resolve the dis-agreement is made and the recommendation is forwarded to the Superintendent of Power.

If the recommendations can not be resalved, the document is forwarded to the Superintendent

  • PROC. NO: PS0 #28 7.3.6 Following review by the Responsible Individu-al and concurrence by the Department Super-intendent, the Superintendent of Power reviews the recommendation.

If the Super-intendent of Power disagrees with the recommendation, the document is returned to I

the PPRS for a second review, evaluation and formulation of recommendations as required in paragraphs 7.3.1 through 7.3.5 above.

7.3.7 The PPRS will select site ors appropriate for industry release on the criteria of event significance or generic impact potential. The event description will be routed to the SDP for accuracy verification and release authorization.

A copy of the final document will be retained with the OER.

7.4 Approval and Action Assignment 7.4.1 Following review, the Superintendent of Power approves the recommendations, assigns action and a due date to implement the recommenda-tion. This action assignment includes one or more of the followings a.

Assignment to one or more Department Superintendent (s) to initiate procedure changes, modifications and so on.

b.

Assignment to the Training Superintend-ent to initiate training associated with the procedure change or modification as appropriate.

c.

Assignment to the PPRS to initiate notification of other utilities via NETWORK or other means as appropriate.

7.5.3 The PPRS closes the item in the Operating Experience Status Log and forwards a copy of the OER form (and attachments if required) to the Manager of Operational Evaluation.

Those items which were determined to be not applicable to JAFNPP are also closed by the PPRS following approval by the Superintendent of Power and closing of the ARMS open item by the Resident Manager's Office.

7.6 Periodic Reports l

7.6.1 The PPRS shall review the Operating Experi-i ence Status Log monthly and note his review by a line entry on the log.

7.6.2 Copies of the Operating Experience Status Log shall be distributed for review by the personnel listed below, at the intervals listed.
