Appendix NOTICE OF VIOLATION The Cleveland Electric Illuminating Docket No. 50-440 Company (CEI)
As a result of the inspection conducted on June 22 through August 2, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violations were identifiec:
1.
10 CFC 50, Appendix E, Criterion V;--es implemented by CEI's Corporate Nuclear Quality Assurance Program (CNQAP), Section 0500 Revisior 6, states that ' activities affecting quality shall be prescribed by oocumented instructions, procedures, or drawings, of a type appropriate tc tne circumstances.'

Values or tolerances prescribed fo-Turbine Control Vaive Fast Ciosure Trip Setocint and Turbine First 5tage Pressure Low Power Setpoint were incorrect in TP 1C71-P001.
In addition, tnis procedure did not provide for restoration / replacement of certain fuses removed to initiate annunciators as required by Test Prograr Instruction (TPI)-7 and TPI-1E.
TP IC71-P002 dic not test interlocks whicn prevent both RPS buses from being suppliec simultaneously fron their alternate power source.
TP 1M51-P001 dic not have permanent plant instrumentation, used to provide acceptance criteria data, designated with the symbol "(AC)" on the Instrument Summary Sheet.
This symbol indicates those instruments which require verification that they have been calibratec within six months of their use ano post-test calibratior verificaticn as reauired Dy TPI-7 anc the Instrument Summary Sneet (440/85042-01(DRS)).
This is a Severity Level It violation (5upplement II).

Criterion XI\\, as implemented through the licensee's CNOAP, Sectior. 1400, reauires that measures be established tc indicate the status of individual items of a nuciear power plant.

Appendix
Contrary to the above, the licensee f ailed to establish adequate measures to insure that the Shift Test Engineer was aware of the status of his system, with respect to temporary alterations, while it is undergoing testing and to thereby insure that the required testing demonstrated that the structures, systems and components will perform satisf actorily in service (440/85042-07(DRS)).
This is a Severity Level IV violation (Supplement II).
