Dear Cleve Reasoner:

On June 30, 2025, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Wolf Creek Generating Station. On July 8, 2025, the NRC inspectors discussed the results of this inspection with Jamie McCoy, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Wolf Creek Generating Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Wolf Creek Generating Station began the inspection period at full power. On April 10, 2025, the unit commenced a down power to 60 percent reactor power at the request of the grid operator due to the loss of the 345 kV Rose Hill line. The unit returned to full power on April 11, 2025, where it remained for the rest of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed onsite portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

The exercise scenario involved hostile action events, including simulated site impact by small aircraft. This resulted in a rapid escalation to a Site Area Emergency classification; activation of alternative response facilities, coordination to move plant staff into the plant for damage assessment with law enforcement; response actions to fuel damage and steam generator rupture; and a simulated radiological release offsite.

Failure to Perform Adequate Audit of the Integrated Public Alert Warning System URI 05000482/2024003-01 71114.02

Description:

During an inspection conducted August 5-9, 2024, the inspectors identified a deviation between the audit practices implemented by the licensee compared to what is described in their FEMA approved ANS design report and its implementing procedures. Therefore, inspectors documented the apparent deviation between audit practices implemented and the design report description in Unresolved Item 05000482/20204003-02 (ML24295A379). On November 22, 2024, the NRC requested a review by FEMA to establish whether the licensee was in compliance with the ANS requirements (ML24306A176).

On February 10, 2025, FEMA's response (ML25042A621) indicated agreement that the documentation available supporting the associated audit report was not sufficient to support the report's conclusions. Input from the Policy and Oversight Branch within the Office of Nuclear Security & Incident Response following this communication indicated that the design and responsibilities associated with the Integrated Public Alert Warning System (IPAWS)program audit are being re-assessed in consultation with FEMA. Therefore, assessment of licensee performance related to audit practices will be paused until later notice. This has no effect on the NRC's established reasonable assurance position (ML25160A112).

Feedback provided in FEMA's assessment was provided to licensee staff on June 4, 2025. Actions based on the feedback will be documented in the corrective action program.

Corrective Action Reference(s): Condition Report 10035589 Observation: Semiannual Trend Review 71152S The inspectors reviewed the licensees corrective action program, performance indicators, system health reports, and other documentation available to help identify performance trends that might indicate the existence of a more significant safety issue. The inspectors verified that the licensee was taking corrective actions to address the identified adverse trends. The inspectors did not review any cross-cutting themes because none existed at the site.

To verify that the licensee was taking corrective actions to address identified adverse trends that might indicate the existence of a more significant safety issue, the inspectors reviewed the corrective action program documentation associated with a potential adverse trend in procedure and work order quality. Specifically, the inspectors reviewed the licensees process through the corrective action program and procedure change process when a deficiency of a procedure or work instruction was identified. From this review the inspectors identified the following examples that did not contain interim corrective action or timely in the implementation of the procedure prior to its next use:

1. Condition Reports 10041256 and 10041490: Condition Report 10041256 documented

a train B class 1E 4160 volt switchgear (NB02) undervoltage relay was found outside technical specification allowed limits during the performance of Procedure STS IC-208B, 4kV Loss of Voltage and Degraded Voltage Trip Actuating Device Operational Test NB02 Bus, Revision 7. A change to Procedure STS IC-208B was still open to adjust the as-left setpoints for the undervoltage relays to account for relay drift. The licensee concluded the condition report owner did not assign a corrective action to change Procedure STS IC-208B, which would have prevented the error from happening. The inspectors did not identify an interim corrective action to adjust the setpoint prior to revising Procedure STS IC-208B with the updated setpoints or placing the procedure on hold until it was updated prior to its next performance.

2. Condition Report 10038872: This condition report documented a minor inspector

identified violation of Technical Specification 5.4.1.d, Fire Protection Program Implementation, for the licensees failure to implement the fire protection program. Specifically, the licensee failed to identify a halon boundary door did not meet the criteria in accordance with Procedure STN FP-440, Fire Door Visual Inspection, Revision 19, due to the door seal not in contact with the threshold area.

The licensees corrective action was to update Procedure STN FP-440 to provide the method for measuring halon boundary door gaps and provide the acceptance criteria to ensure halon boundary functionality. However, the inspectors identified the corrective action was due September 25, 2025, and there were no interim corrective actions to provide guidance to the individuals in the field prior to the update to Procedure STN FP-440.

From the above, the inspectors questioned the procedure change process had sufficient guidance to ensure procedure changes were implemented in a timely fashion prior to the next performance or ensure interim guidance was provided until the procedure update. The licensee initiated Condition Report 10042726 to capture the inspectors observations.
