oV APPENDIX A NOTICE OF VIOLATION Duke Power Company License Nos. CPPR-116 Catawba
& CPPR-117 Based on the NRC inspection May 27-29, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by 10 CFR 50 Appendix B Criterion V and implemented by the Catawba PSAR paragraph 17.1.5.2 " Activities affecting quality shall be prescribed by documented instructions, procedures...and shall be accom-J j
plished in accordance with those instructions, procedures..." The PSAR Paragraph 8.3.1.1.6.b states "All interconnecting wiring and cabling will be inspected for proper installation and connections" and Procedure M40 Revision 6, dated July 6, 1979 states in paragraph 2, "This procedure covers the installation and documentation of all nuclear safety-related cables.
The Duke Power Company Topical QA Program Report 1A states in Table 17.0-1 that the program will conform to Regulatory Guide (RG) 1.30 which endorse ANSI N45.2.4.
ANSI N45.2.4, Paragraph 5.1.1 states in part
" Inspections to verify correctness of installation... shall include...
verificatio'n of... tightness of connections... and terminations."
Contrary to the above, on May 27, 1980, the following items were identified:

Procedure M40 was not being implemented. However, Supplemental Inspec-tion Instruction (SSI) M41B serial 15, Revision 0 dated August 28, 1979, is being used. The SSI M41B serial 15 states that "The frequency

of the termination inspection shall be weekly for a period of not less than one-half of a work shift. This procedure downgrades the termination inspection program below the requirements of M40 and the PSAR in that

all cables will not receive a termination inspection.

SSI M41B Serial 15 states in Paragraph 3 " Visually verify that the J
cable armor and shields have been properly terminated and grounded in j
accordance with CNS 1390.01-00-0035. This inspection shall be limited i
to that which can be observed after the cable has been terminated I
without dismantleing the connector." The tightness of connections and terminations cannot be verified I y this method. This procedure is inadequate.
This is an infraction.

Duke Power Company Appendix A License Nos. CPPR-116 Notice of Violation and CPPR-117 B.
As required by 10 CFR 50, Appendix B, Criterion XI and implemented by the Duke Power Company Topical QA Program Report 1-A, "a test program shall be established... and performed in accordance with written test procedures...
test results shall be documented and evaluated...." The Topical Report states in Table 17.0-1 that the program will conform to RG 1.30 which endorses ANSI N45.2.4. ANSI N45.2.4, Paragraph 5.2 states in part " Surveil-lance of construction activities shall include tests performed in accordance with written test procedures.
Contrary to the above, on May 27, 1980, the licensee was performing a circuit test without a procedure.
This is an infraction.
