Exit Interview The inspection scope and findings were summarized on September 7,1979, with those persons indicated in Paragraph I above. The item of noncompliance identified in paragraph 6 was discussed in detail.

Independent Inspection Effort The inspector observed the calibration of two Magnaflux magnetic particle test sets. The units were tested in accordance with Construction Procedure 33, Revision 1, dated December 7, 1973. The test sets were rejected because poor connections prevented the units reaching the required amperage value.

The licensee reworked the connectors and completed calibration on one unit.

The second unit would not meet the required calibration values and was nonconformed per Procedure Q-1.

Within the areas examined there were no items of noncompliance identified.

Electrical Components and Systems, Observation of Work and Work Activities Two 4160 Volt electrical motors, nuclear service water pump 2A and centrifugal charging pump 2B, and three 480 volt electrical motors, diesel generator (DG) intercooler jacket water pump 2A and DG starting air compressors 2A1

  • and 2A2, were-examined to assure that the requirements of the QAM and QC procedures are. complied with in the areas of receiving inspection, storage, identification, handling, 2nstallation and inspection.

Four electric valve operator motors 2ND67B, 2NDG8A, 2NV221A and 2NV222B were examined to assure that the QAM and QC procedures requirements were complied with in the areas of receiving inspection, storage, handling, identification, installation and inspection.

It was noted that additional bracing was installed to support a large cantilevered connection box on the centrifugal charging' pump motor. The bracket was welded to the motor casting and the connector box. The inspector questioned the licensee about the bracing.

The licensee informed the inspector that the modifications were made by memo from the electrical design group. The inspector noted that the motor was safety related, and thus required approved drawings prior to installing any modifications.

Further investigation identified six additional motors modified without approved drawings. The licensee gave the inspector shutdown request 477, Request for Sh'atdown 1523 and a memo dated October 24, 1977 from C. J. Wylie to J. C. Rogers which requested construction provide a schedule for the work. There was no QC inspection performed on the work as both shutdown forms waived QO.

The inspector identified this item as an Infraction 79-18-01 " Modification to Safety Related Motors Without Approved Drawings."

Electrical Components and Systems Review of Quality Records The equipment identified in paragraph 6 was selected for a quality records review to assure that the specification and QAM requirements were complied with in the areas of receiving inspection, material certification, storage, handliag, identification, storage inspection, cleanliness, installation, installation inspection and post installation protection.

Within the areas examined there were no items of noncompliance identified.

8.

Electrical Components and Systems I Observation of Work and Work Activities The emergency diesel generator 2A, air starting controls and the generator exciter control panel were examined to assure that the requirement of the QAM and QC procedures were complied with in the areas of handling, instal-lation, location, protection, separation, inspection, inspector qualification and post installation protection.

Within the areas examined there were no items of noncompliance identified.

The inspector reviewed Audit I-1-2-79 which had been completed and approved.
