] Diesel Generator Foundation - Record Review

, The inspector reviewed records pertaining to foundation excavation for the

! diesel generator building. Excavation activities in determining competent

! rock capable of supporting a foundation load of 30 KSF were recorded by a

!

Bechtel qualified geologist from San Francisco. His full-time presence extended from February through April, 1986 to approve all rock prior to concrete placement.

!

The geologist's field report contains details of the rock surfaces, in-

,

stallation of steel piles and lagging to protect the excavation and ad-jacent structures, and approval of rock after excavation of soft roc Concrete QC records also contain the geologist's approval of rock for concrete placement to procee The records were observed to meet QC re-quirements for the building foundation as identified on engineering draw-ing C-6PO.

l The geologist's report was observed to contain extensive evidence reflec-ting the qualitative characteristics of the stratigraphic constituents of the sedimentary bedrock encountered and their capability in supporting the diesel generator building.

Additionally, the inspector noted that a geologic map of the rock founda-tion for the Unit 2 diesel generator building would be included in an amendment to the FSA No violations were identifie . Observation of Diesel Generator Building (DGB) Structural Steel and Concrete Construction Structural steel support construction and concrete placement activities for the diesel generator building (DGB) were witnessed. Field construc-

tion engineering personnel and QC engineers were observed actively engaged throughout these activitie The controls exercised were noted to fulfill 3 designated hold points and sign-offs required by the below listed QC l inspection procedures:

-

QCI C-1.10 Inspection of Grouting and Drypacking

'

)

-

QCI C-1.30 Concrete Placement Inspection

The inspector specifically observed DGB constructicn and discussed with '

,

licensee representative, contractor field engineers and quality control

,

engineers details relating to: the structural steel building columns iden-

! tified as "Q" on the West Wall; installation of reinforcing steel; fill-

crete placement as exterior DGB backfill; and, concrete placement, testing j and controls for cells A and Additionally, concrete supplied by the off-site batch plant at Gilbert-sville, Pennsylvania, was inspected and evaluated. The batch plant and delivery fleet of truck mixes were observed to be certified by the NRMCA

'

as required by ACI 301, 197 The batching and truck mixing were contin-uously controlled by a qualified BC batch plant inspector. Concrete trucks were observed transporting the mixed concrete from the batch plant to the site at agitating drum speed. Controls at the site for tempera-ture, slump, air content and compression test-cylinders were observed to be enforced by BC's concrete test inspectors under the site test labora-

, tory superviso No violations were identi fied.

Record Review of Diesel Generator Building Concrete Construction Control Records

<

I Concrete construction QC records were reviewed and evaluated for confor-mance to criteria identified in specification C-36, Forming, Placing, Finishing and Curing Concrete and C-61, Furnishing and Deliver of Off-Site Concrete. Inspection reports for the DGB concrete wall footings, concrete

'

'

pour number DF-F-2 of April 3,1986, were observed to provide the speci-fied controls indicated in QCI C-1.30 for in process inspection activities

and for concrete test cylinder compression test records. Additional

supplementary records for control of the batch plant and transit truck mixers verified the adequacy of imposed control No violations were identified.
Licensee Administrative Controls and Quality Assurance A review was performed of licensee records of the administrative controls

,

and quality a:surance actions pertaining to construction of the diesel generator building. The following surveillance and audit reports were

'

reviewed and discussed with cognizant licensee QA engineers:

'

-

Audit Report #249, DGB Excavation January 29 to March 18, 1086

-

AR #252, Cadwelding Rebar for the DGB, in-9rocess

-

Surveillance Check Report #286, Mudmat Concrete Placement for the DGB, February 26-27, 1986

,

j The above licensee QA activities were observed to contain check lists of specific designated requireme7ts/methodr. of verffication and included objective evidence of the auditors' observations. A report finding of a minor unsatisfactory condition was identified; appropriate follow-up i

action was found to have been taken.

'

No violations were identifie ' Summary Tne NRC inspector concluded from the proceeding observations that con-struction activities for the DGB were being catisfactorily controlle The licensee's QA program appeared adequate in the areas and activities observed.

'i E.vit Interview

, An exit interview was conducted May 23, 1986, at the Limerick plant site

with members of the licensee's staff and contractor personnel identified in paragraph 1. The inspector summarized the scope and findings of this inspection. The licensee acknowledged the inspector's comments. No written information was given to the licensee during the course of the inspection and no information discussed during the exit meeting was found to contain proprietary informatio i
