INSPECTION REPORT NOS. 50-259/89-27, 50-260/89-27, AND 50-296/89-27 Our letter of November 16, 1990, stated that we were requesting that the NRR technical staff review your denial of Example 2 of Violation B.
This example was for failure to submit a Licensee Event Report (LER) within 30 days after discovery of the event concerning the single failure of fire pump lockout relays.
The staff concluded that there was insufficient basis available to support TVA's engineering judgement that the postulated relay failure scenario would only trip one diesel generator.
The ability of some systems to fulfilltheir safety functions was indeterminate during certain plant accident conditions and should have been reported in accordance with 10 CFR 50.73.
A voluntary LER (89-09)
was submitted after the
day requirement and committed TVA to install an additional lockout relay prior to restart of Unit 2.
The staff finds that the corrective action for the conditions described in LER 89-09 is adequate.
Since the plant modifications are now complete, no further response is required.
