This special, announced inspection was in the areas of IEB 80-11 and the snubber surveillance program.

Results:

No violations or deviations were identified.

The results of this inspection did not disclose any particular strengths or weaknesses in the areas inspected.

The licensee's snubber surveillance program met the, Technical Specification requirements.'

The inspector reviewed the procedures listed below which control the snubber surveillance program.

Acceptance criteria utilized by the inspector appear in Technical Specification 3/4.6.H.

Procedures examined were as follows:

Procedure number O-SI-4.6.H. 1, Visual'xamination of Hydraulic and Mechanical Snubbers b.

Procedure number O-SI-4.6.H.2, Functional Test of Hydraulic and Mechanical Snubbers The inspector concluded that the above listed procedures cover the Technical Specification snubber visual inspection and functional test requirements.

Within the areas inspected, violations or deviations were not identified.

IE Bulletin 80-11, Masonry Wall Design, Units 5, 2, and 3 (25537)

a.

Summary of Licensee's Response to IE Bulletin 80-11 The licensee submitted their 60-day response to IE Bulletin 80-11 for Browns Ferry to NRC Region II on July 8, 1980.

This response summarized the acceptance criteria to be used in the masonry wall design evaluation, discussed the schedule for completion of the

masonry wall design re-evaluation, and included data sheets for walls in the proximity of safety-related equipment.

TVA submitted their 180 day IE Bulletin 80-11 response to NRC Region II on November 7,

1980.

This response discussed the methods used to identify masonry walls in proximity of safety-related equip-ment, discussed the function and types of masonry walls at Browns Ferry, discussed the practices employed in construction of the walls and included a detailed discussion of the re-evaluation criteria.

The response also identified the walls which required modification to comply with the design criteria.

On March 4, 1981, the licensee informed NRC Region II that their reevaluation criteria did not take into consideration all applicable loadings.

TVA submitted interim reports on this problem to NRC in letters dated April 7, 1981 and July 27, 1981.

TVA submitted a final report on this problem on October 1, 1981.

This report clarified information previously submitted on IEB 80-11 and included new data resulting from the evaluation of all applicable loadings acting on the walls.

l In a letter dated Januar y 28, 1983, the NRC Office of Nucl ear Reactor Regulation (NRR)

submitted a

request for additional information

.concerning the licensee's IEB 80-11 masonry wall re-evaluation criteria.

TVA responded to this information request in a report attached to a letter dated April 22, 1983.

NRR issued a Safety Evaluation Report in a letter dated January 16, 1984 which accepted, with the exception of four walls, (wall-numbers 3,

40, 74, and 75), the licensee's masonry wall design and concluded that the masonry walls at Browns Ferry in the proximity of safety related equipment would withstand the specified design loads.

NRR requested that the licensee re-evaluate the four unacceptable walls and advise NRC of the proposed plans to upgrade these walls.

The licensee responded to this request in a letter dated June 5,

1984 and provided the results of the reanalysis.

The licensee stated that no safety-related equipment was located within the proximity of these four walls, and that administrative actions would be taken to ensure that no safety related equipment would be located in the proximity of the walls in the future.

This response was accepted by NRR in a

letter dated June 27, 1984.

On October 1, 1984, the licensee informed NRC Region II that a

nonconforming condition had been identified regarding the effects of tornado depressurization on block walls in the Reactor and Control Bay Buildings at the site.

This item was reported as LER 259-84-030 and involved errors in determination of the tornado depressurization loads on the block walls.

In a letter dated Oecember 17, 1984, the licensee reported that they had determined that, as a result of review of the nonconformance, seven additional walls required modification to withstand design loads.

These were in addition to

the

masonry walls identified in the licensee's previous submittals.

Review of Masonry Wall Construction Records In the licensee's November 7, 1980 response to IEB 80-11, the licensee summarized the construction practices used in construction of the masonry walls.

The response stated that tests were made at the time of construction to ensure that all materials used in construction of the masonry walls met design requirements.

The licensee was unable to locate the records documenting these tests for the inspector's review during the current inspection.

Licensee engineers indicated that additional research was needed to locate these records.

The inspector wi 11 examine the records in a future inspection.

This was identified as inspector followup item 259, 260 and 296/89-02-01, Review guality Records Documenting Original Construction of Masonry Walls.

Inspection of Completed Structural Modifications to Masonry Walls As stated above, the licensee determined that

masonry walls requi.red modification to comply with design requirements (19 in original IEB 80-11 analysis p'lus 7 additional due to error in calculation of tornado depressurization loads).

The inspector examined the modifications which wete installed on wall numbers 48, 52, and 60 in Unit 2.

These modifications were installed under Work Plan (WP) 2141-85.

The inspector walked down the walls and verified that the modifications had been installed on Wall 48 and on the outside of Walls 52 and 60 in accordance with the details shown on the design documents.

The inspector examined records documenting inspection of the completed modifications, including visual inspection of welds, and inspection of concrete expansion anchors.

During the walkdown, the inspector also examined modifications which were in the process of being installed on various walls in the Units I and 3 control bays.

The inspector also examined clip angles which had been installed on Unit 2 wall numbers 63'nd 64 under Work Plan 0247-88.

These angles had been omitted during original construction.

This problem was discovered during review for generic implications of an NCR written at the Sequoyah site to document and disposition a similar problem.

The inspector questioned licensee engineers regarding the status of modifications of masonry walls required to meet IEB 80-11 requirements.

The engineers stated that all Unit 2 wall modification had been completed with work on Unit I and 3 walls still in progress.

However, review of correspondence and various documentation disclosed that some walls in structures coranon to Unit 2 and either/or Units 1 and 3 may require modifications which have not yet been implemented (e.g.

wall 106 in the Units I and

Diesel Building).

In addition, the inspector could not determine what type of administrative controls were applied to address TVA commitments regarding wall numbers 3, 40, 74, and 75.

The licensee committed to provide this information for the inspector's review

prior to startup of Unit 2.

This item was identified as Inspector Followup Item 259, 260, and 296/89-02-02, Review Status of IEB 80-11 Analysis and Modifications.

IEB 80-11 will remain open pending further review by NRC.

Within the areas inspected, no deviations or violations were identified.

Inspector Followup Item 259, 260, and 296/83-17-01, Revi.sion of IE Bulletin 80-11 Data Sheets:

The licensee performed a review of the data sheets completed in 1980 and 1981 to respond to IEB 80-11.

This review was performed in accordance with Project Instruction PI 86-40, Plant Walkdown of Masonry Block Walls.

This procedure was written to control the walkdown which was conducted to update the data collected in the original IEB 80-11 walkdown.

The walkdowns identified safety related equipment in the proximity of (i.e. within the Fall zones)

of masonry block walls.

The inspector examined the revised data sheets and noted that the data sheets clearly indicated which block walls were in proximity of safety-related equipment.

During review of the revi sed data sheets the inspector noted that the licensee identified additional walls that were in proximity of safety related equipment.

These walls required removal, repair, or requalification to remain in place and meet IEB 80-11 requirements.

Licensee engineers indicated that these walls had been evaluated and found to be acceptable.

The inspector will review the analyses which qualified the walls as part of the close out of IFI 89-02-02 discussed above.

IFI 259, 260, 296/83-17-01 is closed.

Exit Interview The inspection scope and results were summarized on January 6,

1989, with those persons indicated in paragraph 1.

The inspector described the areas inspected and discussed in detail the inspection results listed below.
