Follow-up on Previously Identified Item (80-18-01) (Closed)
i Through discussions with licensee personnel, and review of procedures and records, the insoector determined the following:

a.
Maintenance Procedure 5-I-2.8, " Shock and Sway Arrestor

Inspection," has been replaced with several new procedures.
b.
The new procedures correct some of the deficiencies noted in iten 80-18-01, namely recording of maintenance corrective actions (the remaining deficiencies are discussed in Paragraph 3, below).

Surveillance of Pipe Support and Restraint Systems The inspector reviewed the procedures governing the surveillance, maintenance, and testing of the pipe supports and restraint systens.
Based on this review, the inspector determined that several new procedures affecting this area had been approved and implemented since the last inspection. This was done to fulfull a commitment to the NRC to revise Maintenance Procedure S-I-2.8 which needed clarification in several areas, including definition of acceptance criteria and testing requirements.
Despite issuance of these revised procedures, the acceptance criteria for non-adjustable snubbers (which were previously poorly defined) were still not clear, in that no numerical values were specified for acceptable lockup and bleed rates.
As a result of discussion of this subject with the licensee, the licensee's representative has agreed to establish inproved acceptance criteria within 90 days. This matter will be verified during a future inspection.
(81-33-01 )
Regarding visual inspection of hydraulic snubbers, the licensee's representative committed to add a statement to Maintenance Procedures 501-I-2.30 and 2.32, "that an unsatisfactory oil level (below complete coverage of the intake port in the reservoir) constitutes an inoperable snubber which must be renoved and bencn tested to determine operability." The inspection indicated that such a criterion was inadequate in that it did not provide a margin for level changes during heatup or for leakage which occurs between inspections.
The inspector also discussed this matter with.the licensee and the licensee's representative conmitted to provide within 90 days a minimum reservoir oil level acceptance criterion based on the individual type of snubber. The inspector will review this commitment during a future inspection.
(81-33-02)
The inspector visually examined approximately 40 of the 117 accessible and inaccessible snubbers (both hydraulic and nechanical).
Of the hydraulic, 5 were identified by the licensee's representative as low on oil and one, 1-SW-393-2, was found by the inspector to have an empty reservoir. These snubbers were removed durinn the insoection for bench testing. The results of this testing will be examined at a subsequent inspection.
(81-33-03) No deficiencies were identified by the inspector in the remainder of the snubbers and pipe support and restraint systems which were observed.
In order to verify that the required snubber surveillance, maintenance and testing had been perfarned, the inspector requested the records of these activities.
The licensee, however, could not provide these in a timely fashion (the records were provided two days later

Since no time remained for examination of these records, this item will be examined at a subsequent inspection. (81-33-04) At the exit meeting, the inspector noted the difficulty in obtaining the records in a timely fashion.
The inspector observed the calibration of the snubber bench tester and determined that the equipment used for calibration was within its required calibration frequency. The inspector also determined the qualifications of,the testina supervisory personnel appeared to conform tosthe guidance contained in Regulatory Guide 1.58.

No items of noncompliance or deviations were identifed.
4.
Independent Inspection Effort During in-containment inspection of pipe supports, the inspector observed unused sheets of plastic and other debris on the. floor.
Since these materials could block the safety-related containment sump in the event of an accident, the inspector informed the licensee's representative of this condition.
In response, the licensee's representative committed to remove this debris prior to startup.
No itens of noncompliance or deviations were identified.
5.
Exit Meeting The inspector met with the licensee's representatives (denoted in Paragraph 1) at the conclusion of tne inspection on October 2, 1981. The results of the inspection were discussed and the licensee's representatives provided commitments as noted in Paragraphs 3 and 4.
