Failure to Close Fire Penetration AECM-83/0798 On or about March 28, 1983, fire penetration CE-519D located in the Computer Room on elevation 148 feet of the Control Building was opened to implement a Design Change Package (DCF). This penetration was opened under a Maintenance Work Order (MWO) to install conduits per the Design Change Package.

An MWO was written to close the penetration but the required design closure detail was not included in the DCP package. Therefore, penetration CE-519D was deleted from the closure MWO. The MWO was completed and closed. The MWO to open the penetration remains open. An LCO was entered for the work order but CE-519D was inadvertently left off the LCO due to the large number of penetrations (approximately 23) on the work order. Although no LCO report identified the penetration as being covered by the required fire watch, a fire watch was in effect for the affected room from March until November 10 under other LCOs with the exception of October 16.

On November 10, 1983, Material Nonconformance Report 01005-83 was issued to generate the appropriate action. An LCO was entered and a fire watch was established in accordance with Technical Specification 3.7.7 within one (1) hour and will continue until the penetration is sealed.

To preclude further occurrence, programmatic controls to require notification of and authorization by the Shift Superintendent / Supervisor prior to opening a penetration were implemented on September 24, 1983.

AECM-83/0798 MISSISSIPPI POWER Q Ll3HT COMPANY Page 2 penetration. This Special Report is submitted pursuant to Technical Specification 6.9.2 as the seven (7) day limit of Technical Specification 3.7.7 was exceeded. The penetration is expected to be sealed by February 28, 1984.
