Grand Gulf Nuclear Station Unit 1 Docket No. 50-416 License No. NPF-29 Special Report 88-005/0 Accident Monitor Inoperable for More Than 72 Hours AECM-88/0181 On August 29,1988 at 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br />, the Post-Accident Monitoring instrument for the Standby Gas Treatment "B" subsystem exhaust was declued inoperable when the intermediate range noble gas activity channel failed a source check.

The check source is electrically inserted on demand via a remote computer comand tenninal . The source count rate moniter is actuated by a limit switch on the check source mechanism. The check source failed to produce a count rate on demand due to a failed limit switch.

There wera no qutlified replacement parts for the failed limit switch on-site.

The time required to procure an environmentally qualified replacement part caused the monitor to be inoperable for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. Action 81 of Technical Specification 3.3.7.5 requires the initiation of a preplanned alternate method of monitoring if the instrument is inoperable for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

Procedure 06-CH-1D17-V-0058, "Ventilation Exhaust Accident Range Alternate Noble Gas, lodine, and Particulate Monitoring" requires grab samples to be obtained if certain plant conditions are met. Since the nonnal effluent monitoring instrumentation was functional during the time period, grab samples were not required to be taken.

The monitor was restored to service on September 3 at 1400 after obtaining and installing a qualified replacement switch. Qualified spare limit switches have been procured and placed in the plant warehouse stock. This 14-day Special Report is submitted pursuant to Action 81 of Technical Specification 3.3.7.5 due to the instrument being inoperable for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.
