Crand Gulf Nuclear Station Unit 1 Docket No. 50-416 License No. NPF-29 Special Report 89-002/0 Inoperability of Accident Monitor Exceeds 72 Hours AECM-89/0056 A Turbine Building ventilation exhaust accident range radiation monitor was removed from service on February 25, 1989 at 0600 to perform a surveillance ,

as required by Technical Specification Table 4.3.7.5-1. Channels 3 and 4 -I failed and channel 4 remained in check source.

An investigation into the problem revealed the following defective components: flow switch, channel 4 check source mechanism, interface box, and detector' cable. The detector cable was reworked and the remaining defective components were replaced. The monitor retested satisfactorily and was restored to service on March 2, 1989 at 1550.

This Special Report is being submitted pursuant to Action 81 of Technical Specification Table 3.3.7.5-1 since the instrument channel inoperability exceeded 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. The preplanned alternate method of monitoring as required by Action 81 is to utilize the Eberline Spring Monitoring System per procedure 08-S-04-220.  !

The effluent monitoring instrumentation was operable during this time period.
