Reactor Coolant System Fluoride CONDITIONS PRIOR TO OCCURRENCE:

DESCRIPTION OF OCCURRENCE:

On 2-28-78, it was reported that the reactor coolant system fluoride sample which was due on 2-25-78 was not taken.

This was discovered by chemistry personnel when the next routine sample was taken on 2-27-78.

DESIGNATION OF APPARENT CAUSE. OF' OCCURRENCE::

This occurrence was caused by a personnel error in that the Technical Assistant failed to identify the required fluoride sample on the chemistry schedule.

ANALYSTS OF OCCURRENCE:

Although the required fluoride sample was not taken, samples taken prior to and following the 2-25-78 date were within the steady state limit.

All other required reactor coolant system chemistry samples were also within the steady state limit.

CORRECTIVE ACTION

To prevent reoccurrence of this type of event, a copy of the chemistry schedule will be posted in the chemistry lab.

This posting will clarify the required chemistry samples and due dates.
