Non-Radwaste Sampling for Suspended Solids..

CONDITIONS PRIOR* To OCCURRENCE:

N/A DESCRIPTION OF OCCURRENCE:

On the occasions summarized below, treated effluent was released from the non-radioactive liquid waste treatment basin, but a sample was not collected for suspended solids analysis.

June 24, 25, 26, 1978.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was personnel error.

The responsible technician* failed to take the required suspended solids samples.

ANALYSIS OF OCCURRENCE:

N/A

CORRECTIVE ACTION

To* prevent reoccurrence of this type of event, a daily check list is being developed that will require all samples to be signed off and reviewed by a supervisor.

This system is expected to be fully imple~

mented by August 15, 1978.
