Inoperable Air Lock Inner Door CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 DESCRIPTION OF OCCURRENCE:

At 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br />, Health Physics personnel exited containment after taking air samples.
Upon exit, surveillance 4.6.1.3.a was performed to verify elevation 130' air lock leak rate.
Test showed excessive leakage on inner door.
The air lock was declared inoperable and Action Statement 3.6.1.3 was implemented.
The unit was in Mode 3 at this time in preparation for a cooldown to Mode 5.
At 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br /> the Shift Supervisor sent an operator to investigate the air lock.
The operator removed a small piece of foreign material and tested the inner door leak rate.
Results were satisfactory.
Action Statement 3.6.1.3 was terminated at 2217 hours0.0257 days <br />0.616 hours <br />0.00367 weeks <br />8.435685e-4 months <br />.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
This occurrence was caused by a 1/2 inch long section of 1/16 inch plastic tubing being trapped in the door seal.
This foreign material did not allow the door to seal completely causing the excessive leakage.
ANALYSIS OF OCCURRENCE:
Technical Specification 3.6.1.3 states that with an air lock inoperable, restore the air lock to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least Hot Standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in Cold Shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.
The elevation 130' inner door was returned to operable status within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and 15 minutes.
The unit was in a cooldown to Mode 5 during this period.
CORRECTIVE ACTION
Investigation by Operations personnel revealed a small piece of foreign material trapped on the door seal.
The material was removed and the leakage test rerun.
Results confirmed operability.
