SL1-25 was the tenth refueling outage following installation of the Babcock &Wilcox Advanced Series PWR replacement steam generators (SGs) in 1997. At the SL1-25 refueling outage, the replacement SGs had accumulated approximately 13.85 EFPY (166.20 EFPM) ofcumulative operation since replacement in 1997. Following the SL1-25 refueling outage, St. Lucie Unit1 initially entered HOT SHUTDOWN (Mode 4) on November 1, 2013 @ 05:18, which marked thebeginning of the SL1 -25 operating cycle. Therefore, the report is due to the NRC by May 01, 2014.The SL1-25 SG tube inspections were conducted in both 'A' and 'B' SGs as described below and meetTechnical Specification (TS) Section 6.8.4.1.d.2 inspection requirements for the first half of the secondsequential period of 108 months.a. The scope of inspections performed on each SGThe scope of this examination is summarized in Table 1 below and is established to meet thefollowing requirements:

a. Technical Specification 6.8.4.1 "Steam Generator (SG) Program"b. Technical Specification 6.8.4.1.d.2 "Inspect 100% of the tubes at sequential periods of 144, 108, 72 and, thereafter, 60 effective full power months. The first sequential period shall be considered to begin after the first in-service inspection of the SGs. In addition, inspect 50% of the tubes by the refueling outagenearest the midpoint of the period and the remaining 50% by the refueling outages nearestthe end of the period. No SG shall operate for more than 72 effective full power months orthree refueling outages (whichever is less) without being inspected".

7. EPRI, Palo Alto, CA: 2007. 1013706.The basis for bobbin and +PointTM tube examinations is given in Table 1. Applicable industry qualified eddy current examination techniques are provided in Table 2.Diagnostic rotating probe examinations (Special
interest, SI) were completed as required based on theresults of the bobbin coil.Prior to starting the eddy current examination at the SL1-25 refueling outage in 2013, all of thepreviously installed hot-leg and cold-leg plugs in Steam Generator "A" and Steam Generator "B" wereconfirmed to be present in their correct locations and were confirmed to be free from degradation andvisible signs of leakage based on the visual examination.

The plant TS requirement (TS Section 6.8.4.1.d.2) to inspect 50% of the tubes by the refueling outagenearest the midpoint of the 2nd 1 SI period of 108 months has been satisfied by the October 2013inspection.

  • Sludge Lancing* FOSAR (Includes post sludge lancing visual assessment and foreign object retrieval)

There are currently no known foreign objects remaining in the secondary side of the steam generators that require tracking.

Supplemental Primary Side Inspections to address Westinghouse NSAL 12-01Issue

Description:

Visual inspection of the SGs at a foreign Westinghouse designed plant showed indications ofdegradation in the cladding and/or divider plate-to-channel head weld, with exposure and wastage ofthe channel head base material.

Westinghouse issued NSAL 12-01 "Steam Generator Channel HeadDegradation" on January 5, 2012 and included recommended actions for the visual inspection at otherplants.Although the degradation is not credible for St. Lucie Unit 1, a visual inspection of the primary sidechannel head bowl was performed in accordance with the requirements presented in Westinghouse NSAL 12-01.Results:Based on the visual inspections performed, no visible signs of degradation or other abnormalconditions were identified.
