12PMP4021.TRPB001Theinitialeventonsignoffsheet6.1Rxistheinitiatingeventforthereactortrip.Thiscouldbesteamgeneratorlo-lolevelorturbinetripaboveP-7,forexample.Thissignoffsheetshallbecompletedforeveryreactortrip.Theinitialeventonsignoffsheet6.1TURistheinitiatingeventfortheturbinetrip.Thiscouldbesteamgeneratorhi-hilevelorturbinetripfromreactortrip.Thissignoffsheetshallbecompletedfoeveryreactortripwhichoccurswiththeturbinereset.Theinitial.eventonsignoff6.1GENistheinitiatingeventforthegeneratortrip.Thiscouldbeturbinestopvalvesclosedandgeneratormotoringcauseoveralldifferentialtrip.Thissignoffshallbecompletedforeveryreactortripwhichoccurswiththegeneratorexciterfieldbreakerclosed.Ifexpectedresponseswerenotobtainedorwereoutsidetheacceptabletimeframe,andthetriphadinitially,beendeemedaConditionIevent,thenthetripnolongersatisfiesthecriteriaforaConditionIeventandthe.full-lengthperformanceofthisprocedureisrequired.NOTE:Reactorrestartmaynottakeplaceuntilthefailureiscorrectedifthefailureissafetyrelated.Signoffsheet6.1includesasignoff(signoffsheetstep6.1.3)todocumentSSandSTAconcurrenceforacceptabilityofrestartforthosetripswhichareclassifiedasConditionIeventsinsection4.0ofthisprocedure.CompletionofthissignoffissufficienttorecommendrestartoftheaffectedunittothePlantManagerforConditionIevents.NOTE:ThePlantManager'sdecisiontorestartisdocumentedinOHP4021.001.002.Page8of15Rev.0 12PMP4021.TRP.0016.1.7IftheeventisbeingreviewedasaConditionIevent,completesignoffsheet6.3,ReviewReportcoversheet,andattachtoitallsignoffsheetscompletedintheConditionI'eventreview.SeeStep6.2.10fordistributionofthisreviewpackage.IThisconcludestheConditionItripreview.6.2INVESTIGATIONINTOTHECAUSEOFTHETRIPNOTE:Troubleshootinganddiagnosisareactivitieswhichcannotbereadilydescribedinaprocedure.Thefollowingstepsincludeobjectives,,authorizationtoobtainadditionalassistance,directionforputtingthedatain.aformwhichmayleadtounderstanding,andinstructionstoverifycertainautomaticresponsesnotaddressedinsection6.1.Judgementispermittedintheimplementationofthissection.Theproceduralstepsaretobeusedasa-guide.Alternatetechniquesmaybeemployedtoanalyzetheeventaslongastheendresultisanunderstandingofthecauseoftheevent,verificationthatrequiredautomaticresponsestookplace,andidentificationofany,detrimentaleffectsontheplantorequipment.6.2.1IfthetriphasnotbeenclassifiedaConditionIevent,afullinvestigationofthesituationiscalledfor.ToreleasetheSStofulfillhisotherobligations,thisfunctionwillnormallybecarriedoutbytheOperationsSuperintendent,orhisdesignee.Afteraddressingperformanceofthedatagatheringand"verificationofresponse"portionsofthisprocedure,theSSshallcontacttheOperationsSuperintendentandrequestthatheassumeresponsibilityforcompletionoftheinvestigation.TheSSmayathis~otion,performthefunctionsoftheOperationsSuperintendentlistedbelow,relinquishingtheresponsibilityiftheinvestigationbecomestooinvolvedorhisattentionsarerequiredforotherfunctions.6.2.2Thepurposeoftheinvestigationistodeterminethecauseofthetripandassesstheplant'sreadinesstoreturntopower.Ifatanypointintheinvestigation,theOperationsSuperintendentconcludesthatheisunabletoachievethepurposeoftheinvestigationPage9of15Rev.0 Untilthenewinformationsystemisinplaceandoperational,wewillcontinuetoreviewandupdatetheexistingsystemasrequiredandconductrefreshertrainingforappropriateplantstaff.ThenewsystemisdescribedinSection2.2.1.6.2.2.1.3PlantUseUnderthepresentsystemofworkcontrol,ajoborderispreparedforallrepairandmodificationworkperformedattheplant.Duringthejoborderpreparationprocess,thesafetyclassificationoftheequipment,aswellasproceduresrequiredtoperformthework,areenteredonthejoborderform.Thesamereferencedocumentsareusedtoassurethatproperlycertifiedreplacementpartsareusedwhenrequired.Plantprocedurescontroltheorderingofreplacementparts.Followingcompletionofeveryrepairandmodification,thejobordersundergoareviewprocessbyexperiencedandknowledgeablesupervisorypersonnel.Thisreviewprocessprovidesverificationthattheinformationhandlingsystemisbeingusedonaroutinebasis.Particuiarattentionisgiventosafetyrelatedjoborderstoverifythatproperprocedureswereusedandthatproperlycertifiedreplacementpartswereinstalledwhererequired.2.2.1.4ManagementControlsTheactivitiesdescribedaboveforworkcontrolprocessareaddressedinvariousplantinstructionsandprocedures.TheQualityAssuranceDepartmentperformsauditsofactivitiescoveredbytheplantinstructionsandproceduresandnotifiesappropriateplantmanagementofanydeficienciesnoted.Thus,theauditprogramprovidesadditionalverificationoftheroutineutilizationoftheinformationhandlingsystern.2.2.1.5WearecurrentlyengagedinthecompletionofthequalificationofcomponentsofsafetyrelatedsystemslocatedinharshenviroauentsassociatedwithIEBulletin79%1B.Thisprogramincludesconsiderationofagingofthesedevices.Rule10CFR50.49requiresthatreplacementcomponentsforsafetyrelatedsystemsmeettheconditionsofIEEE323-1974whichincludesagingrequirements.SpecificcriteriaforexemptionsareprovidedbyRule10CFR50.49.

Ourcurrentspecificationsusedtoprocureneworreplacementcomponentsidentifynormalandaccidentserviceconditionsorreferenceapplicablecodes~Qualificationtestingandperformanceevaluationisrequiredforharshenvironmentsandtestreportsofthisqualificationtestingarerequiredtomeettheconditionsofthespecifications.Priortofuture-use-forprocurement,eachspecificationsubjecttotherequirementsof10CPR50.49willberevisedtoincludethe'requirements'hatthevendorestablishservicelifebytestorperformanceevaluation=andrequirethevendortosupplydocumentationinsupportoftheservicelifequalification.NewClassificationProgramWeplantoimplementanew.computerizedcomponentclassificationrecordwhichvilllistallplantcomponents,theirsafetyclassificationandtheirprocurementandQArequirements.Crossmeferencestodrawingsandrelevantplantandcorporateprocedureswillensurethatthesafetyroleofacomponentiskeptinfocuswheneveracomponentistakenoutofservice,bought,maintained,replacedorreturnedtoservice.Thejobcontrolclassificationwillrelyontherecord.Werecognizetheneedtoaddresstheissueof"importanttosafety"andtotieittospecificcomponentsandtheirapplications.ThissubjectisbeingactivelydebatedwithintheindustryandisthesubjectofMr.DarrelEisenhutsGenericLetter84&1datedJanuary5,1984.Atthepresenttime,however,thedefinitionofthescopeoftheterm"importanttosafety"hasnotbeenestablished;wedonothaveformalcriteriatoallowclassificationonacomponentbycomponentbasis.Weareplanningournewcomponentclassificationrecordtoaccommodatesuchclassifications.Asadditionalrequirementsaredevelopedforthe"importanttosafety"classifiedequipmentthesewillbereviewedand,ifappropriate,incorporatedintotheprogram.Wearestartingworkonournewdata>>baseprojectsWecurrentlyexpecttohavethenewsysteminplacebeforetheendof1986.

3.2POSTMAINTENANCETESTING(ALLOTHERSAFETY-REIATEDCOMPONENTS)3.2.1Wehavereviewedourplanttestingandmaintenanceprocedureswhichcoversafetyrelatedcomponentsandhavedeterminedthatpostmaintenancetestingisincluded.Weareconductingreviewstoverifythatthetestingspecifiedisadequatetodemonstratethattheequipmentiscapableofperformingitssafetyfunction.ThisreviewisexpectedtobecompletedbyMarch31,1985.Planttagoutproceduresrequirethatoperabilityisdemonstratedpriortoreturningthecomponentorsystemtoservice.Componentswithinthetagoutsboundaryarereviewedforoperabilityverification.Plantproceduresgoverningjobordersrequireadequatetestinguponcompletionofthejob.Thetestingperformedbythedepartmentcompletingtheworkhastobeenteredonthejoborder.Whenachangeismadetoaplantstructure,system,orcomponent,therequestforchange(RFC)packageisreviewedbythedesignchangecoordinator.RFCinstallationsarenormallyrequiredtobetestedtoverifyoperability.Ifsuchatestisnottobeperformed,theLeadEngineerhastodocumentthejustificationfortheexception.Operabilitytestingwhichiscomplexisperformedbyusinganapprovedprocedure.TheD.C.CookNuclearPlanttechnicalspecificationsarethestandardizedformat.However,theydonotspecificallyrequirethatpost~aintenancetestingbeconductedbeforereturningasystemorcomponenttoservice(i.e.,declaredoperable).Specifiedsurveillancetest(s)(operabilitydenenstration)arerequiredpriortoentryintoanoperationalmode.Inaddition,whentheplantisinaconditionthatrequiresentryintoa"TechnicalSpecificationActionStatement",therelevantplanttagout,joborderandchangecontrolproceduresareactivated.Thisensuresthattheoperabilityofallsafetyrelatedcomponentsisverifiedbeforetheyarereturnedtoservice.TheplantcanthenbeconsideredtobeoutoftheActionStatementcondition.3.2.2Wehavecanmencedareviewoftheplantmaintenanceprocedures.Thecheckforvendorandengineeringrecommendationstodeterminethatappropriatetestguidanceisincludedwillbeincludedinthereview.Inaddition,aseparatereviewisbeingmadeofthetechnicalbulletinsfortheWestinghousesuppliedNSSSsafetyrelatedequipmentasdefinedbytheWestinghousebulletin.Thissamereviewwillbemadeonothersafetymelateddevicebulletinsastheyare'eceived (includingpreviouslyissuedbulletins)undertheprogramdescribedbySection2.2.2.InconjunctionwithourVDCS,wewillgiveyouastatusreportontheresultsofourreviewbyDecember31,1985.

ATTAQRENTIII 2.2.2ThefollowingdescriptionexpandsourNovember4,1983letterAEP:NRC:0838,VendorDocumentControlSystemandprovidesanimplementstionscheduleGeneralZnordertoensurethatvendorinformationforsafetymelatedcomponentsiscomplete,current,andcontrolledthroughoutthelifeoftheD.C.CookNuclearPlant,a"VendorDocumentControlSystem"(VDCS)hasbeendevelopedandisintheinitialstagesofimplementation.TheVDCSiscomprisedofacorporatelevelgeneralprocedureandassociatedreviewdocumentationforms,andisdesignedtotrackVendorTechnicalDocuments(VTDs)fromtheirreceipttofinaldispositionandfiling,utilizingpositivefeedbackateachstagetoguaranteereceiptandon-schedulereview.ToassureimplementationofandadherencetotheVDCS,theAEPSCQualityAssurancesectionhas.beenassignedtheresponsibilityformonitoringandauditingthegeneralprocedure.VDCSOb'ectivesPriortothedetaileddevelopmentoftheVDCS,itwasessentialthatprogramobjectivesbewelldefined.Theresultingobjectiveswererealistic,achievable,andaddressedtheintentofGenericLetter83-28,Section2.2.2.Theseobjectivesandthemannerinwhichtheywereaddressedaredescribedasfollows:1.CentralizationofResponsibilityforVTDsBecausethevendorsusesuchdiversemethodsfortrananittingVTDstoAEP,theassigningtoonecentralgrouptheresponsibilityforreceiving,classifying,anddeterminingdistributionforallVTDswasaprimerequirement.TheNuclearOperationsSection(NOS)withintheAEPSCNuclearEngineeringDivisionhasbeenassignedthisresponsibility.AllVTDspertainingtoD.C.CookPlant,regardlessoftheirpointsofarrivalwithintheAEPSystem,willbeforwardedtotheNOSforinitialprocessing.2.TraceabilityofallVTDsTheabilitytotraceandlocateaVTD,notonlyduringtheinitialreviewprocessbutatanytimeduringthelifeoftheplant,willbeachievedthroughtheassignmentofuniqueprocessingandfilingnumbers.Uponreceipt,theNOSwillassigntoeachVTDauniqueprocessingnumber.Thisnumberwillberetainedfrominitialsortingthroughfinaldispositionandfiling.DuringthereviewoftheVTDbythecognizantdivisions,allVDCSformswillbeidentifiedwiththisuniqueprocessingnumber.Duringthereview,iftheVTD(s)isdeterminedtobe"applicable",itwillbeassignedauniquecontrolnumber.A"ControlledDocument"stampwillbeaffixedtotheVTD,andthecontrolnumberprintedthereon.Boththeprocessingnumberandthecontrolnumberalongwithallotherpertinentdata,willberecordedintheVDCS.Thissystem-(detailedlater)willenabletheVTDtobelocatedanditsstatusdetermined.

SystemtoRecordandTrackVTDDataBecauseofthelargenumberofVTDsbothexistingandprojectedtobereceivedinthefuture,acomputerizedtrackingsystemisunderdevelopment.ThissystemwillenableallVTDstobetrackedateachpointofthereviewprocessfrominitialreceiptthroughfinaldispositionandfiling.AlldatapertinenttotheVTDandthecomponenttowhichitisapplicablewillbeincluded.CompleteDocumentationforallVTDsThedocumentationrequiredforthereceipt,review,anddispositioningofeachVTDisprescribedbytheVDCScorporatelevelgeneralprocedure.Eachstepofthereview'processrequirestheuseofaformspecificallydevelopedforthatstep.Allformscontainaseparate"receipt/acknowledgement"sectionwhichaustbecompleted,signed,andreturnedtotheoriginator.TheVTD,alongwithalldispositionforms,willbemicrofilmedandfiledatthecompletionofthereviewprocess.TimelyReviewandDeterminationofRequiredActionsThetimeallottedtothecognizantdivisionsforreviewonaVTDisspecifiedbytheNOSonthetransmittalformattachedtotheVTD.AannthlyreviewofVDCSitemswillbeconductedbytheNOS.TheNOSwillinitiatean"overduenotice"toacognizantdivisionand/oradjustcompletiondateswhennecessary.AllVTDswillbeprocessedthroughtheVDCS.However,ifaVTDindicatesthatimmediateactionisrequired,andtheprocessingdelaythroughtheVDCScouldadverselyaffecteitherthehealthandwelfareofthegeneralpublicorCookPlantoperations,thegeneralprocedurepermitstherequiredactiontobetakenwithoutimmediateVDCSprocessingandreview.Aftertheactionsarecompleted,thegeneralprocedureprescribesthattheVTDnmstbeprocessedthroughtheVDCSinthenormalmanner~AbilitytoRetainandRetrieveallVTDsandAssociatedMaterialUponcompletion'ofthereviewprocess,theVTDandallassociateddocumentationwillbemicrofilmedforpermanentstorageandallpertinentinformationrecordedintheVDCStrackingsystem.WheneverinformationisrequiredconcerningaVTD,asearchofthecomputerizedtrackingsystemcanbemade.OncetheVTDislocated,itmaybeviewedonmicrofilmand/orreproducedasrequired.EnsureReceiptandCorrectProcessingofallVTDsToensurethatallVTDstransmittedtoAEPbyvendorsarereceived,theVDCScontainsprovisionsforaperiodiccontactwitheachsafetymelatedvendor.Overanygiventwelve(12)monthperiod,eachvendorwillbesentaformletter,includinga"receipt/acknowledgement"section,withanattachmentlistingthedocumentsreceivedfromthevendorduringtheprevioustwelve(12)aonthperiod.Ifthislistingiscorrect,the vendorwillberequestedtosignandreturnthedocument.Ifdocumentsaremissing,thevendorwillberequestedtosoinformAEPandprovidecopiesofallmissingdocuments.TheseVTDs,uponreceipt,willbeprocessedthroughtheVDCS.ProcessingofallVTDSincludesanevaluationof"applicability"toD;C;CookPlant."Applicable"VTDswillbeautomaticallyprocessedthroughtheVDCS.However,ifaVTDisclassifiedas"notapplicable"bythe'NOS,itwillbeautomaticallysenttoacognizantengineeringgroupforasecondevaluationofapplicability.Thisstepreducesthepossibilityofan"applicable"documentbeingmisclassifiedandnotbeingreviewedandcorrectlydispositioned.8.IncorporationofExistingVTDsintoSystemTheincorporationofexistingVTDsintothenewVDCSisunderway.Eachdocumentwillbereviewedandassignedauniqueprocessingnumber.EachVTDapplicabletoD.C.CookPlantwillbeclassifiedandstampedasa"ControlledDocument"andassignedacontrolnumber.AllpertinentdatawillberecordedintheVDCStrackingsystem.NUTACAEPactivelyparticipatedintheNuclearUtilityTaskActionCanmittee(NUZAC)onNRCGenericLetter83-28,Section2.2.2-VendorInterface.WhileAEPisingeneralagreementwiththeNUXACrecommendations(transmittedtotheNRCbyE.Griffing,NUZACChairman),wehavemodifiedtheprogramslightlytomeetourspecificrequirements.VDCSCurrentStatusandImlementationScheduleSubsequenttothereceiptofGenericLetter83-28,anintenseefforthasbeenunderwaywithinAEPtodevelopandimplementaprogramtoaddressthedocumentcontrolconcernscontainedtherein.AEPiscanmittedtothesuccessfulandon-scheduleimplementationoftheVDCS.Thefollowingscheduleiscontingentupontheobjectivepreviouslydefinedremainingunchangedthroughthetargetedcompletiondates.Additionalregulatoryrequirementsorunforeseeabledifficultiesarisingfromthefull-scaleimplementationoftheVDCSgeneralprocedurecouldcausedelays.Becausetheimplementationscheduleisheavilydependentupontheperformanceofthevendors,thefollowingaretargetsdatesratherthancommitment.dates.1.VDCSGeneralProcedureThecorporatelevelgeneralprocedureandassociateddocumentsdetailingthehandlingandreviewprocessforallVTDshasbeencompletedandforwardedtoAEPseniormanagementforreviewandapproval.Anticipatingapproval,implementationisproceedingandtheVDCSisexpectedtobeoperationalwiththecompletionofthecoayonentclassificationrecorddescribedunder2.2.1.6.

2.ReactorTripSystem(RTS)ComponentVendorsAllRTSvendorshavebeencontacted,andresponsesarebeingevaluatedfordeterminationofrequiredactions.Asthecompletionofthisactivityisalmosttotallydependentuponavendorresponseitisnotpossibletoprojectacompletiondate.However,thetimelinessofresponsesisbeingmonitored,andthosevendorswhoarelatearebeingrecontacted.3~Safety-RelatedCoaponentVendorsAEPisintheprocessofcontactingallsuppliersofsafety-relatedcomponentsandrequestingalistoftechnicaldocuments(VTDs)pertainingtoD.C.CookPlant.Becauseofthedepthoftheinformationwhichmustbegatheredandsuppliedtothevendorforeachcomponent,thiswillbealengthyundertaking.ItisanticipatedthatallsafetymelatedvendorswillbecontactedandnewdocumentsreviewedandincorporatedintotheVDCSbythetargeteddateofDecember31,1985.Astheresponsesarereceived,theyareevaluatedfordeterminationofrequiredactions.4.IncorporationofExistingVTDsintotheVDCSTheexistingsystemformonitoringandtrackingVTDsismuchlesssophisticatedandincorporatingtheseexistingVTDsintotheVDCSdescribedhereinrequiresacomprehensiveinventoryoftheCookPlantmasterfiles.ThiseffortisunderwayandisanticipatedtobecompletedbythetargeteddateofSeptember1984.TransferoftheseVTDsintotheVDCSwilltakeapproximatelynine(9)monthsbeyondthis.

ATTACHMENTIVIAmendmentstopages7,23,and24ofoursubmittalAEP:NRC:0838AdatedNovember4,1983 ThecomplementofprogramsfortheP250processcomputerincludestwoprogramswhicharerelevanttotheanalysisofreactortrips.ThesetwoprogramsarethePostTripReviewProgramandtheSequenceofEventsRecordingProgramTheSequenceofEventsRecordingProgramrecordsthesequenceofoperationofanumberofmonitoredcontactstoahightimeresolution.Whenoneofthemonitoredcontactschangesstate,aninterruptisinitiatedwhichcausestheP250toscaneachmonitoredcontactforanychangefromitspreviousstate.Theprogramstoressuchchangesandthecyclecountsincethefirstevent.Acycleisnominally20millisecondsinlength.Duetoadeadtimeof2millisecondsintheinterruptprocess,anautanaticrebidoftheprogramisprogrammedforthecyclefollowingeachinterruptbid.Thisisdonetoavoidlossofcontactchangesduringthedeadtime.TheSequenceofEventsRecordingProgramisterminatedwheneitherthecyclecountreaches3600or25contactchangeshavebeenrecorded.Whentheprogramisterminated,anoutputroutineiscalled.AllcollecteddataarefirstmovedtotheoutputprogrambufferstofreetheSequenceofEventsRecordingProgrambuffersforcontinuedmonitoring.Theoutputroutineprintsthetimeofthefirsteventinhours,minutes,andseconds.Followingthismessage,thealphanumericaddress,a36-charactercontactdescription,andcyclecountfromthefirsteventareprintedfor"eachcontactchange.Thefirsteventwillalwayshaveacyclecountofizero.TheP250addresslistindicatesthatthereisaninputtotheSequenceofEventsRecordingProgramforeachpotentialreactortrip.Inthecaseofreactorcoolantpumpunderfrequency,partialtripsarealsoincluded.Inaddition,thereactortripandreactortripbypasscircuitbreakers,maingeneratoroutputcircuitbreakers,andturbinestopvalvesaremonitored'hetimediscriminationbetweeneventsisonecycleornaninally20milliseconds.Theformatfordatadisplayisdiscussedaboveinthedescriptionoftheprogram.ThedataisoutputononeoftheP250typewriters.Theprinteroutputsheetsmayberetainedforfuturereference.TheprimarypowersourcefortheP250computerisaninvertersuppliedbytheABbatteryand600voltbusllB.Iftheinvertershouldfail,theP250computerwouldbeswitchedbyanautanaticbustransfertothecontrolroompowerdistributioncircuit,CRP-3,whichissuppliedfromtheplantlightingtransformer.Thepowersourceisbalanceofplant(nonclassIE).

4..5SstemFunctionalTesting4.5.1.STAandUVTATestinThereactortripbreakersarecurrentlytestedon-linebyoperationoftheundervoltagetripdevice~Thepresentarrangementoffourcircuitbreakers,twotripandtwobypass,permitson-linetestingofthebreakers.WecurrentlytesttheUVTApriortoeverystart-upandonceeverymonthduringunitoperation.Theshunttripisindependentlytestedpriortoeachstart-up.Nofailureshavebeenencountered.Oncethemodificationtoenableautomaticactuationoftheshunttripattachmentismade,wewilltestboththeUVTAandtheSTAwhiletheunitisinoperation.4.5.2.On-lineTestinSinceweperformon-linetesting,thissectiondoesnotapplytoourplant.4.5.3.FrequencofOn-lineTestinOurpositionisasfollows'.ThereactortripcircuitbreakersattheD.C.CockPlantareinstalledinacleananddrylocationandarenotsubjecttoanydeleteriousenvironmentalinfluences.Thepresentmaintenanceprogramrequiresthatthecircuitbreakersbeservicedateveryrefuelingoutage.Atthistimethemechanicalfeaturesofthecircuitbreakersareinspectedandadjustedasnecessarytomaintainthecriticalclearancesdeterminedbythemanufacturertobenecessaryforreliableoperation.Thecircuitbreakeranditscompartmentarecleanedandlubricationisappliedasrecommendedbythemanufacturer.Themaincontactresistanceisverifiedtobeacceptablebytest.Thecircuitbreakersaretheninstalledinthemetalcladenclosures.Priorto.returningthecircuitbreakertoservice,anelectricalfunctionaltestisperformedwhichteststheelectricalclosing,electricalshunttripandtheundervoltagetrip.IncompliancewiththeTechnicalSpecifications,theundervoltagetripofeachcircuitbreakerhasbeentestedonlineatmonthlyintervals.Therehavebeennofailuresofthereactortripcircuitbreakerstotripduringtestsorinactualoperation.Thishistoryofexcellentperformancehasbeenmaintainedforover9yearsforoneunitand6yearsfortheother.Thepresentsurveillanceandmaintenanceprogramhasbeenadequate,resultinginnofailures.Thepresentmaintenanceschedulepermitsinspectionsandadjustmentstothecircuitbreakersatintervalswhicharemorefrequentthannecessary,consideringthecleanenviroanentandlightelectricalservicerequiredofthecircuitbreakers.Thetestingatonemonthintervalshasprovidedthenecessaryexercisetoensurefreedomofmotionofthecircuitbreakersandits attachmentswhentheyarecalledontooperatet'operformtheirsafetyfunction.Assumingamaxiaamtestintervalofonehourforeachon-linesurveillancetestforeachcircuitbreaker,the1reactorprotectionsystemisdependentononesafetytrainfortrippingfortwohourseverymonth.Increasedon-linetestingfrequencywillresultingreatertimedependencyononetrainfortrippingwithoutincreasingtheassurancethatthereliabilityofthecircuitbreakershasbeenimproved.TheWestinghouseOwnersGroupiscarryingouttestsonthesebreakers.WeexpecttoreceivetheirresultsinFebruary1984.Ifthereareanychangeswefeelweneedtomakeasaresultofthetests,wewillcommunicatetoyoubyMarch30,1984.
